Endothelial damage and subsequent clotting is common in severe and critical COVID-19 coronavirus, which may have implications for treatment, Chinese clinicians said at a webinar co-sponsored by the Chinese Cardiovascular Association and American College of Cardiology.
"Clots in the small vessels of all organs, not only the lungs but also including the heart, the liver, and the kidney," were described by Bin Cao, MD, of the National Clinical Research Center for Respiratory Diseases in Beijing, who helped develop treatment strategies there from the beginning of the epidemic.
His group had reported March 11 in The Lancet that D-dimer levels over 1 μg/L at admission predicted an 18-fold increase in odds of dying before discharge among 191 COVID-19 patients seen at two hospitals in Wuhan, China.
D-dimer, a fibrin degradation product indicating thrombosis, can exceed 70 or 80 μg/L, he said
"Anticoagulation therapy should be initiated for severe COVID-19 patients [unless] otherwise contraindicated," said Cao, who is also president-elect of the Chinese Society of Respiratory Medicine.
"If you see a high D-dimer like that, you have to give anticoagulation, regardless of the underlying mechanism," agreed webinar moderator Junbo Ge, MD, president of the Chinese Cardiovascular Association and ACC China chapter governor.
However, Harlan Krumholz, MD, of Yale University, called for first testing whether anticoagulation helps.
"What's most important for us is to develop the means to study what works," he said on the webinar. "There will be many plausible approaches to the care of these patients."
"Our Chinese colleagues are reporting to us a very high rate of cardiac involvement," he continued. "I think most cardiologists in the U.S. are still thinking this is not a cardiac disease, this is something else. ... But saying to our colleagues that this does involve the heart is one of the central messages we're hearing."
Another was that physicians in the U.S. should expect the unexpected from COVID-19.
In China, "we had a very hard time, especially in the early period," said Ge. "Today we try to share our experience in the last three months. We try to help our colleagues ... to shorten the mistakes that we had in China and especially for the patients with multi-organ damage."
"I have to remind American doctors: normal life is changing," said Cao. "If you think you should live your normal life, it's not true. Normal life is changing, not only in China but in America and other countries. ... It's a pandemic life now."
The Mechanism of Cardiac Injury
Acute cardiac injury was reported in 12% of COVID-19 cases in a small case series in The Lancet cited by Cao. Another study suggested a rate of 7.2% among 138 patients from another hospital in Wuhan, noted Yundai Chen, MD, of the Chinese PLA General Hospital in Beijing, during the webinar.
Comorbid cardiovascular disease is a distinct risk factor for COVID-19, associated with a mortality rate of up to 10.5% among more than 70,000 patients in one study, she pointed out.
The SARS-CoV-2 virus that causes COVID-19 disease enters cells via the angiotensin converting enzyme 2 (ACE2) receptors, which is most commonly found in the alveolar epithelial cells, followed by endothelial cells, Cao noted.
"The virus can bind to the endothelial cells and may cause damage to the blood vessel especially the microcirculation of the small blood vessels," which leads to platelet aggregation, he said. "You can imagine that it is not a myocardial infection, it is not a stroke, it is the clots all over the body. So you can imagine why the high D-dimer. It is because of the wide spread of abnormal coagulation all over the body."
Along with endothelial shedding and thrombosis in vessels, autopsies showed inflammatory changes in the heart with fine interstitial mononuclear inflammatory infiltrates, but no viral inclusions in the heart, Chen added. Other potential mechanisms for the cardiac damage are hypoxia-induced myocardial injury, cardiac microvascular damage, and systemic inflammatory response syndrome.
However, in the majority of mild and moderate cases, the only cardiac impact was some tachycardia and little higher troponin, Chen noted. "Maybe preventing the transition to severe cases is crucial."
For severe COVID-19 patients with heart dysfunction, the strategy is to treat the symptoms, prevent complications, treat the pre-existing disease, and provide functional support in time, Chen said. Key points she highlighted were:
  • Respiratory support with extracorporeal membrane oxygenation provided in time for ICU patients with acute heart failure
  • Circulation support with sufficient fluid resuscitation, paying attention to fluid balance
  • Continuous renal replacement therapy to protect the kidneys provided as early as possible
For severe or critical cases, monitoring ECG may turn up tachycardia, generally with a heart rate of 100 or 120 bpm, Chen noted. "It is quite dangerous for initiating acute coronary syndrome or acute myocardial infarction, so we need to also pay attention to this."
Notably, fulminant myocarditis from the disease can mimic ST-segment elevation myocardial infarction (STEMI).
Chen pointed to the case of a COVID-19-infected man with apparent STEMI by ECG, troponin T over 10,000 ng/L, and CK-MB of 113 ng/L, but for whom coronary angiography showed no stenosis. After treatment with steroids, immunoglobulin, norepinephrine, diuretic, a vasodilator, and antibiotics, the man's ejection fraction recovered from 27% to 66% and his enlarged heart normalized.
Fulminant myocarditis is rare, though, Ge noted. "The main issue is hypoxia," he said, because it makes myocardial ischemia more severe.
As far as treatment of pre-existing cardiovascular disease, management is not special, Chen said. ACE inhibitor and angiotensin receptor blocker (ARB) medication use "remains controversial," but those on antiplatelet drugs should stay on them while checking closely for bleeding; liver function should be monitored closely for those on statins, and selective beta-blockers are recommended if there is no hypoxia or airway spasms, she added.
Antiviral drugs used to treat COVID-19 may also interact with cardiovascular drugs -- for instance, liver injury and muscle enzyme elevations if lopinavir/ritonavir (Kaletra) is taken with certain statins, Chen said. Also, there is sudden cardiac death risk with chloroquine, and risk of bradycardia with lopinavir/ritonavir.
Future Implications
The long-term implications of COVID-19 cardiac injury aren't clear, said Ying-Ying Zheng, PhD, presenting in English on behalf of Yi-Tong Ma, MD, PhD, both of the First Affiliated Hospital of Xinjiang Medical University in China, in the webinar.
In 12-year follow-up of a cohort with another type of coronavirus, SARS, there were cardiovascular system abnormalities in 44% of the 25 patients, high lipids in 68%, and glucose metabolism problems in 60%.
"Given that SARS-CoV-2 has a similar structure to SARS-CoV, this novel virus may cause chronic damage to cardiovascular system, and attention should be given to cardiovascular protection during treatment for COVID-19," Zheng said.

Another threat from the lung virus that causes Covid-19 has emerged that may cause swift, sometimes fatal damage: blood clots.
Doctors around the world are noting a raft of clotting-related disorders -- from benign skin lesions on the feet sometimes called Covid toe to life-threatening strokes and blood-vessel blockages. Ominously, if dangerous clots go untreated, they may manifest days to months after respiratory symptoms have resolved.
The clotting phenomenon is probably the most important thing thats emerged over the last perhaps month or two, said Mitchell Levy, chief of pulmonary critical care and sleep medicine at the Warren Albert School of Medicine at Brown University in Providence, Rhode Island.
Its not unusual for infections to raise the risk of clotting. The 1918 Spanish flu pandemic, caused by a novel strain of influenza that killed some 50 million people worldwide, was also linked to downstream damage from clots that could end lives dramatically.
Viruses including HIV, dengue and Ebola are all known to make blood cells prone to clumping. The pro-clotting effect may be even more pronounced in patients with the coronavirus.
Theres something about this virus thats exaggerated that to the nth degree, said Levy, who is also medical director of the medical intensive care unit at Rhode Island Hospital. Were seeing clotting in a way in this illness that we have not seen in the past.
The problem is visible in clots -- doctors call them thrombi -- that form in patients arterial catheters and filters used to support failing kidneys. More pernicious are the clots that impede blood flow in the lungs, causing difficulty breathing.
Rapid Deterioration
These are probably whats causing patients who otherwise appear well to suddenly fall off the ledge and develop severe blood-oxygen deficiency, said Margaret Pisani, an associate professor of medicine at the Yale University School of Medicine in New Haven, Connecticut.
Clotting disorders in Covid-19 patients were noted by researchers in China in February, but their gravity has since become clearer. While doctors had thought the vast majority of lung damage was due to viral pneumonia, theyre now looking more closely at clotting.
When you look at autopsies now, we are seeing things that we didnt expect, said Anthony Fauci, the director of the National Institutes of Allergy and Infectious Diseases whos at the forefront of the U.S. pandemic response. Clumps of platelets inside blood vessels, or microthrombi, are probably why Covid patients can rapidly and dramatically deteriorate, he said in an interview with CNN last week.
Separate studies from France and the Netherlands found that as many as 30% of severely ill Covid-19 patients suffered a so-called pulmonary embolism -- a potentially deadly blockage in one of the arteries of the lungs. These often occur when bits of blood clots from veins deep in the legs travel to the lungs. By comparison, the prevalence of pulmonary embolism was 1.3% in critically ill patients without Covid-19, one study found.
Cardiac Arrest
If untreated, large arterial lung clots can put overwhelming strain on the heart, causing cardiac arrest. Even tiny clots in the capillaries of lung tissue may interrupt blood flow, undermining attempts to help oxygenate patients with ventilators, said Edwin van Beek, chair of clinical radiology at the University of Edinburghs Queens Medical Research Institute.
In the early 1990s, Van Beek helped develop the D-dimer blood test thats used around the world to monitor clot formation in patients, including those with Covid-19, and to dose them with heparin and other anticoagulant medications.
Untreated pulmonary embolism is lethal in one in three cases, and will recur in another third, he said. In 3% to 7% of patients, it will cause pulmonary hypertension, another dangerous complication that can cause fatigue and shortness of breath.
Scarred lungs and clotting-related problems may be a lingering legacy of the pandemic, Van Beek said. Covid-19 survivors who have subsequent difficulty breathing, especially on exertion, might mistakenly believe its a recurrence of coronavirus infection, when it may actually be a reactivation of the whole clotting problem.
I expect to see more of this as we come out of the pandemic, he said. Patients and doctors alike may not be aware of the risks or the potential need for treatment.
Coagulation may occur because of damage to cells lining blood vessels that results from both the viral infection and the immune systems inflammation-causing response, said Jean Connors, a Harvard Medical School hematologist.
The outcome isnt affected if youre treated appropriately, she said. But its possible that people are dying from undiagnosed pulmonary emboli.
Organ Damage
Clots may form in other parts of the body, potentially damaging vital organs including the heart, kidneys, liver, bowel, and other tissues.
Five cases of stroke were treated in Manhattans Mount Sinai Health System over a two-week period through early April, doctors reported in the New England Journal of Medicine last week. The patients, who all had the coronavirus and were younger than 50, were treated for large-vessel blockages.
Its a rare complication amplified by the sheer numbers of infected patients, Connors said. New York City has reported about 170,000 Covid-19 cases, including roughly 43,000 hospitalizations.
Puzzling, Enlightening
Such findings are puzzling on one hand, but on the other hand are enlightening because they can inform better ways to treat patients, said Fauci, the NIAID leader.
In Italy, the first European country gripped by the pandemic, it was after Covid-19 patients died from acute pulmonary emboli and other clotting-related events that doctors moved to inflammation-blocking treatments, such as tocilizumab, sold by Roche Holding AG as Actemra, said Frank Rasulo, a head of neuro critical care at Spedali Civili University Hospital in Brescia.

What we have observed these past few months is the predictable radical right racist medley of blaming the virus on the usual suspects: that it is a ploy by the Jews/the Chinese/Bill Gates/Soros/the American military etc. Radical right conspiracies of Covid-19 come in two kinds, often held simultaneously in the same message: that the virus does not exist and/or there is a manufactured deadly virus because they want to exterminate us.
In the first case, denial of the virus is based on random shreds of evidence such as when people film their area hospital (#filmyourhospital) to show that it is not overwhelmed by Covid-19 patients. In the second case, the virus is planted to weaken the peopleand sometimes to hide the deadly side effects of 5G technology.
It is difficult to document all the Covid-19 conspiracies that have been floating around and even more demanding to account for their existence. Furthermore, the mainstreaming of these theories means that by the time they reach an audience they are stripped of the theory: they are conspiracy lite as Muirhead and Rosenblum aptly describe in their book, A Lot of People are Saying. People share them recklessly, just to show that they are not going to be taken for a ride: we may be locked down, but we know what is really going on.
The current pandemic is also bringing new life to the darkest corners of the extreme right where visions of violence and destruction seem more plausible than ever. The accelerationist ideologylinked to white supremacist killers, encourages events that will sow discord and chaos that will eventually lead to the collapse of governments and the violent uptake of power by the white race.
There are already indications, at least from online rants, that accelerationist ideologues find the current pandemic as a ripe opportunity to pursue this agenda. Contrary to conspiracy theories that view this pandemic as a global-scale prank, accelerationists see it as an opportunity to prove their prophecies and have already used online forums to intensify radicalization efforts. As Cynthia Miller-Idriss has noted, the current epidemic is ripe for exploitation by the far right.
For example, Simon Lindberg, the leader of the Nordic Resistance Movement in Sweden seized the moment to warn (white) followers that the pandemic may lead to a much anticipated catharsis: You might have to sink to the bottom before you can climb to the top. For people to truly realize that the current anti-white system and the corrupt forces ruling us today have to be replaced, they might first need to realize their true nature in full and also understand that there is actually an alternative.

A doctor claims that COVID-19 was present in Europe in December, which is earlier than what was originally thought.
Dr. Yves Cohen, an intensive care chief in Paris, said that looking back, a positive case of COVID-19 was identified at Jean-Verdier Hospital in the northeastern Paris suburb of Bondy on Dec. 27, according to ABC News. That would be around the same time the first cases of the virus were detected in China.
Doctors reanalyzed samples from molecular diagnostic tests of 24 patients in December and January who were experiencing pneumonia-like illnesses but tested negative for the flu and other viruses. At the time, tests for the coronavirus were not yet being offered.
Cohen said the man who tested positive may have been France's "patient zero" and urged doctors to retest all patient samples that had been negative for pneumonia.
France reported its first confirmed case of COVID-19 on Jan. 24, linking it to the southwestern city of Bordeaux. It was also the first reported case in Europe.
There have been 131,000 reported cases of the coronavirus in France, with nearly 25,000 deaths resulting from the virus. More than 50,000 patients who contracted COVID-19 have recovered. Cases across Europe have reached nearly 1.4 million.

JACKSON, Miss. (AP) The key ingredient castor beans is easy to find. Crude instructions for extracting the lethal poison in them can be found on the Internet. And it doesn't require a chemistry degree or sophisticated lab equipment.
The FBI is investigating at least three cases over the past month and a half in which ricin was mailed to President Barack Obama and other public figures.
Ricin has been sent to officials sporadically over the years, but experts say that there seems to be a recent uptick and that copycat attacks made possible by the relative ease of extracting the poison may be the reason.
"I can absolutely promise you that when these kinds of things happen, we're going to have copycats. We expect them. We prepare for them. And we catch them," said Murray Cohen, founder of the Atlanta-based Frontline Foundation, which trains workers in how to respond to bioterrorism and epidemics.
Security and counterterrorism expert Michael Fagel, who teaches at Northwestern University and is a veteran of ricin investigations, said ricin may be employed because castor beans are so easy to come by.
The plants grow wild along highways and in other spots in the U.S. They are also considered ornamental by some gardeners and are cultivated for medicinal castor oil and other products.
"And you can go on the Internet and find out any one of a gazillion recipes on how to make ricin," Fagel said, adding that it takes only a beginner's knowledge of science to "weaponize" it.
If inhaled, ricin can cause respiratory failure, among other symptoms. If swallowed, it can shut down the liver and other organs, resulting in death. The amount of ricin that can fit on the head of a pin is said to be enough to kill an adult if properly prepared. No antidote is available, though researchers are trying to develop one.
Despite the poison's fearsome reputation, a draft of a 2010 Homeland Security Department handbook lists only one person killed by ricin, and that was a 1978 assassination in London involving injection with a ricin pellet. Someone associated with Bulgaria's secret police used a special umbrella to fire the pellet into a Bulgarian dissident.

The first of the three recent ricin investigations in the U.S. began in April. An Elvis impersonator, Kevin Curtis, was jailed and accused of sending poisoned letters to Obama, a U.S. senator and a Mississippi judge. Then Curtis was suddenly released from jail when the FBI shifted its focus to his longtime foe, James Everett Dutschke. He was charged with making ricin.
The FBI said Dutschke, a former martial arts instructor and unsuccessful candidate for various political offices, bought his castor beans on eBay and may have used a coffee grinder to turn them into a powder from recipes he downloaded on his computer.
Then in May, three poison-tainted letters were mailed from Spokane, Wash., to Obama, a federal judge and a post office. A fourth letter sent to Fairchild Air Force Base near Spokane also tested positive for the poison. The FBI is trying to locate a fifth letter it suspects was mailed to the CIA in McLean, Va.
Matthew Ryan Buquet, a 37-year-old janitor and a registered sex offender, was charged last week with mailing a threatening communication. He has pleaded not guilty.
In the most recent case, authorities say ricin-laced letters were sent to New York Mayor Michael Bloomberg and his Washington gun control group. Those letters were postmarked from Shreveport, La., but could have been mailed from Arkansas, Louisiana or Texas, officials said.
Cohen said it appears the ricin in all three cases was a crude form that's relatively easy to make. It would require laboratory equipment and scientific knowledge to make a more potent, weapons-grade version, he said.
Soon after the ricin case in Mississippi, the Congressional Research Service wrote a report for Congress that said recipes for ricin are available "on the Internet, from commercial bookstores, in patents and in scientific literature."
"The quality of these directions varies. Some directions would produce only crude preparations, while others would produce nearly pure ricin," the report said.
Thomas Pittman, a retired professor at the University of Southern Mississippi and an expert in toxicology, said ricin has a number of qualities that make it desirable to someone bent on doing harm: The ingredients are relatively easy to obtain, it's not that hard to make a crude form of it, it's deadly and it's hard to detect in a victim's body.
"It's the perfect poison," he said.
The congressional report noted some ricin cases over the years.
In 2008, authorities said a man in Las Vegas may have accidentally poisoned himself with ricin that he had made from a backyard castor plant. Roger Bergendorff told The Associated Press at the time that he made the ricin just for the sake of having it, and swore he had no intention of harming anyone. He was sentenced to more than three years in prison.
In 2003, someone sent a ricin-laced letter to the Transportation Department that was critical of new rules governing truck drivers. A similar letter was addressed to the White House the following month. Both were intercepted and nobody was hurt.
In 2004, ricin was detected in a letter sent to then-Senate Majority Leader Bill Frist's Washington office.

Is Covid-19 in fact the Mass Ricin Toxicity?! 

The clinical picture: signs and symptoms in the acute illnesses in both conditions is very similar, practically the same: the multiple organ failures with the underlying Coagulopathy, the abnormal and excessive blood clotting in the blood vessels (Disseminated Intravascular Coagulopathy) and in organs: recently described as Pulmonary Intravascular Coagulopathy. 

"Early on in the COVID-19 pandemic, it appeared that the primary risk of death from the disease was severe pneumonia followed by a cytokine storm. As more is discovered about the disease caused by the SARS-CoV-2, an unexpected cause of death has been identifiedheart attacks and strokes caused by blood clots. And this, in particular, appears to be the cause of death in some younger patients under the age of 50.

With regard to ricin, "inhalation of the toxin presents with non-cardiogenic pulmonary edema, diffuse necrotizing pneumonia, interstitial and alveolar inflammation, and edema."

"Collectively, these data suggest that the diffuse bilateral pulmonary inflammation observed in COVID19 is associated with a novel pulmonaryspecific vasculopathy which we have termed pulmonary intravascular coagulopathy (PIC) as distinct to DIC. Given that thrombotic risk is significantly impacted by race, coupled with the accumulating evidence that coagulopathy is important in COVID19 pathogenesis, our findings raise the intriguing possibility that pulmonary vasculopathy may contribute to the unexplained differences that are beginning to emerge highlighting racial susceptibility to COVID19 mortality."

The related common clinical conditions also include necrosis of the liver and kidneys, GI, CNS, dermatological symptoms, etc. 
The major routes of contraction: inhalation and digestion (hypothetical in Covid-19), with direct contact as transitional route, appear to be the same in both conditions. 
The Covid-19 Antibody test is positive in approximately 15% of population across the different locations, and up to 80-90% of the Covid-19 designated cases are asymptomatic.

Ricin is easily obtainable and is solvable in water. The recent cases of the attempted Ricin poisoning in letters mailed to the Government officials, and also the attempt to manufacture the "Ricin bomb" to be used during the Dusseldorf Carnival are alarming and telling: they indicate the high level of interest in using this substance as the biological weapon. 
The tests for detection of Ricin in clinical settings do not exist, therefore the differential diagnosis with Covid-19 in the hospitals cannot be performed.

  • No methods are available for the detection of ricin in biologic fluids.
  • Tests for ricinine, an alkaloidal component of the castor bean plant have been developed.

The existing specialized screening tests are practiced by the FBI at the mailing facilities to detect the threat of the bioterrorism. The potential use of these tests in clinical settings can be considered but their utility and value in clinical settings is questionable.

It is possible, that the Covid-19, including the unprecedented mass media influencing campaign, in effect the information warfare, skillfully conducted in conjunction with the quite realistic but still hypothetical at this point biological warfare, is just a sophisticated cover for the mass Ricin poisonings, including the targeted killings. It might be the well designed and performed Intelligence Operation. Press reports about the situations with Czech mayors and ricin attacks is the case in point, although it looks and feels more like the German - New Anwehr attempt to implicate the Russians and to use them as the cover, by exploiting the Russian militaristic nationalists, rather than the genuine activity of the Russian Intelligence Services. 
The many small scale dry runs preceding this hypothetical Mass Ricin Attack may have well included the mysterious deaths of the American tourists in the Dominican Republic, around the time of the contemplated by the US invasion of Venezuela, possibly as the tool of the intimidation and warning directed at the military personnel of the US bases there. 

This Hypothesis of Covid-19 as the Mass Ricin Toxicity deserves very serious and the utmost attention, and it has to be explored ASAP.

The recent discoveries of ricin, a deadly biologic toxin, at a South Carolina postal facility, a White House mail facility, and a US senator's office has raised concerns among public health officials, physicians, and citizens. Ricin is one of the most potent and lethal substances known, particularly when inhaled. The ease with which the native plant (Ricinus communis) can be obtained and the toxin extracted makes ricin an attractive weapon.

OBJECTIVES:

To summarize the literature on ricin poisoning and provide recommendations based on our best professional judgment for clinicians and public health officials that are faced with deliberate release of ricin into the environment. LITERATURE ACQUISITION: Using PubMed, we searched MEDLINE and OLDMEDLINE databases (January 1950-August 2005). The Chemical and Biological Information Analysis Center database was searched for historical and military literature related to ricin toxicity. Book chapters, unpublished reports, monographs, relevant news reports, and Web material were also reviewed to find nonindexed articles.

RESULTS:

Most literature on ricin poisoning involves castor bean ingestion and experimental animal research. Aerosol release of ricin into the environment or adulteration of food and beverages are pathways to exposure likely to be exploited. Symptoms after ingestion (onset within 12 hours) are nonspecific and may include nausea, vomiting, diarrhea, and abdominal pain and may progress to hypotension, liver failure, renal dysfunction, and death due to multiorgan failure or cardiovascular collapse. Inhalation (onset of symptoms is likely within 8 hours) of ricin is expected to produce cough, dyspnea, arthralgias, and fever and may progress to respiratory distress and death, with few other organ system manifestations. Biological analytic methods for detecting ricin exposure are undergoing investigation and may soon be available through reference laboratories. Testing of environmental samples is available through federal reference laboratories. Currently, no antidote, vaccine, or other specific effective therapy is available for ricin poisoning or prevention. Prompt treatment with supportive care is necessary to limit morbidity and mortality.

CONCLUSION:

Health care workers and public health officials should consider ricin poisoning in patients with gastrointestinal or respiratory tract illness in the setting a credible threat. Poison control centers and public health authorities should be notified of any known illness associated with ricin exposure.

Ricin is one of the most toxic substances known to science with an LD50 between 3 and 30 µg/kg in humans. Potentially lethal when inhaled, ingested, or injected, death can occur within 36 to 72 hours upon exposure. Ricin works by preventing the bodys cells from producing essential proteins, which leads to cellular death. There is no known antidote for ricin poisoning.

Occurring in the kernel of the castor bean, a species indigenous to East Africa which is used to produce castor oil, ricin can be produced from the mash, a by-product of castor oil processing. Stable under normal conditions, ricin can be inactivated by heat over 176 degrees Fahrenheit (80 degrees centigrade). It can be processed into powder, pellet form, mist, or be dissolved in weak acid or water.
Unless the castor bean is chewed and ingested, unintentional ricin poisoning is unlikely, and ricin must be deliberately produced to be able to be used as a poison.

Effects of different routes of ricin poisoning on the body

Depending on the route of exposure and the dose administered, ricin poisoning can have different effects on the body, with differing levels of severity and potential lethality. Initial symptoms can appear after 4-8 hours following exposure, and in some cases up to 24 hours.

Inhalation

Inhalation of significant amounts of ricin leads to respiratory distress, fever, nausea and chest tightness within hours of exposure. Pulmonary edema (the build up of fluid in the lungs) can follow. Eventually, this can lead to respiratory failure which can be fatal.

Ingestion

Ingestion of ricin can lead to vomiting and diarrhea, which potentially could be bloody. This can result in severe dehydration and low blood pressure. Seizures and passing blood in the urine can also be symptoms, and in the most severe cases organ failure (kidney, liver, spleen) can lead to the persons death.

Skin and eye contact

Exposure to the skin and eyes with ricin can cause redness and pain. Whilst it is unlikely that absorption of ricin can occur through normal skin, it occurs that if ricin is on one's hand and food is consumed afterward that the ricin is thereby ingested.
Ricin is of concern to governments and scientists worldwide because of its potential weaponization. Due to its lethality, it has been used offensively, including for the assassination of Georgi Markov, a Bulgarian writer, and dissident in 1978. There have also been reports of its use in Iraq as a chemical weapon. The US Army investigated its application in the 1940s.
Considering the ease with which ricin can be produced, and its potential use as a weapon, studies are ongoing into how to detect and treat ricin intoxication in the event of it being used in these ways. One such area of research is that of metabolomic profiling.

Metabolomic profiling of ricin

Metabolomic profiling is the measurement of multiple small molecule metabolites in biological specimens. This includes tissues, bodily fluids such as blood and urine, and breath exhalate.
In a study by Pingping Guo et al. in 2014, an NMR-based metabolomics approach was used to investigate ricin toxicity. They complemented this with clinical chemistry and histopathological inspection.
The study highlighted several metabolic disturbances caused by intoxication with crude ricin isolated from castor beans, which lead to decreased nutrient uptake, lung and kidney impairment as well as thymus and spleen dysfunction in rats. Long-term crude ricin intoxication produced energy, nitrogen and amino acid metabolism perturbations, as well as evoking oxidation stress.
Guo et al. furthermore found potential biomarkers which could be useful for diagnostic studies of the toxicity of ricin that could explain why ricin has such a high toxicity.
Riccardo V. DElia et al. in 2019 studied the effects of ricin intoxication on Sprague-Dawley rats, and attempted to study metabolic changes over time. In the study, they administered ricin intravenously into the rats and collected blood samples from the animals for up to 48 hours after exposure. They analyzed blood plasma isolated from the samples, applying HILIC and C18 reversed-phase UHPLCMS assays and followed these with multivariate and univariate analysis.
They found that a total of 37 metabolites were altered significantly by ricin intoxication (compared to control groups), with the most significant ones being the triglyceride, bile acid, and arginine/proline metabolic pathways.
The results of these studies helped to demonstrate that metabolites may be useful as a tool to detect and diagnose ricin exposure.
The potential of metabolomic profiling to assist health professionals to provide treatment at a stage early enough after exposure to increase the chance of survival is promising. Until an antidote is found for ricin poisoning, the only effective way to help those poisoned survive is to provide treatment and therapy that counteract symptoms to the best extent possible.

More than a month after a German court convicted a 31-year-old Tunisian man of planning to carry out a ricin attack, his German wife who is accused of helping him, denied the charges on Friday.

In late March, the Dusseldorf regional court found Sief Allah H. guilty of manufacturing a biological weapon and preparing an attack, sentencing him to 10 years in prison.

His 44-year-old wife denied plotting with her husband to carry out a biological bomb attack with the deadly poison ricin.

She also said that she didnt know the dangers posed by ricin that was stored at their apartment in Cologne, and that she would not let the toxic material be near her seven children.

Federal prosecutors said the couple decided in 2017 to detonate an explosive in a large crowd.

The pair had allegedly researched various forms of explosives before deciding on the deadly poison.

They ordered 3,300 castor beans over the internet and successfully made a small amount of ricin.

They also bought a hamster to test the potency of the poison.

The couple were caught in June 2018 after a tip-off from the US Central Intelligence Agency, which had noticed the large online purchase of castor seeds.

Before travelling to Germany, Sief Allah H. worked as a mailman in Tunisia.

He had tried to travel to Syria to fight alongside extremists. But when he failed to go there, he thought about an alternative plan to carry out the biological attack.

In another case, the Dusseldorf regional court sentenced Carla-Josephine S., a 33-year-old German woman, to five years and three months in prison, for taking her three children to Syria in 2015 without the knowledge or consent of their father.

The eldest son, trained as a child soldier aged 7, is thought to have died in a 2018 rocket attack.

She had also joined a women's ISIS unit in Syria.

A covid-19 infection starts as a local upper respiratory tract infection, but can spread to affect multiple organ systems with consequences that are only now being understood. When it does spread in this way, the result is multi-system critical illness associated with a high risk of death.
The multi-system manifestation of a covid-19 infection is caused by a combination of specific host defence responses with associated inflammatory activity and (micro) vascular involvement with distinct coagulopathy and a strong propensity to develop thromboembolic complications. The hyper-inflammatory tissue response, going hand in hand with a compromised circulatory system, leads to fulminant multiple organ dysfunction affecting lungs, heart, kidneys, nerves, muscles, gastrointestinal tract and brain. In the patients most severely affected, a cytokine storm occurs, characterized by very high levels of pro-inflammatory cytokines and Tumour Necrosis Factor (TNF)-α, interleukins, granulocyte-colony stimulating factor, and several chemokines. These patients are those at highest risk of multi-system failure and significant mortalityMales and those with pre-existing hypertension or coronary disease are at higher risk of severe disease, consistent with known correlates of ACE2 expression; the ACE2 gene is located on the X chromosome.
The widespread nature of these effects reflects in large part the ability of the SARS-CoV-2 virus to infect endothelial cells via the ACE2 receptors, with likely release of cytokines that make them more adhesive and increased coagulation; patients with covid-19 can manifest hypercoagulability, with high levels of D-dimer, fibrinogen, and factor VIII, leading to venous thromboembolism and a mixture of large proximal pulmonary artery thrombi and fibrin micro-thrombi in the presence of vascular inflammation. Thus, in the lungs, post-mortem samples from critically ill patients reveal local haemorrhage.
Starting with the cardiovascular system, cardiac enzyme release can be observed even in the early stages, suggesting myocardial inflammation and damage. Cardiologists have reported patients presenting with chest pain as the primary symptoms in the absence of pyrexia and other disease manifestations.
Renal failure is a common complication of severe covid-19. Proximal renal tubular cells express ACE-2 and direct infection by SARS-CoV-2 may cause the renal insult, but indirect injury may occur through endothelial inflammation or as a consequence of medical management. Reduced fluid administration to minimise pulmonary vascular congestion, aggravated by elevated positive end-expiratory pressure as a ventilatory strategy with ARDS, may increase pressure in the renal vein, impairing renal blood flow.
Gut and liver dysfunction, manifest by deranged tests of liver function and slow tolerance of enteral feeding, are well recognised and occur more frequently in severe illness.
Neurologic abnormalities are documented in up to 50% of the most severely ill patients, including impaired consciousness, acute cerebrovascular events, and muscle disease. SARS-CoV-2, may directly invade the central nervous system and neuromuscular tissue both via a vascular route and by retrograde neuronal transmission.
It is in the ICU setting where organ failure in covid-19 is most studied. The April ICNARC data reports 93% patients requiring basic and 25% advanced cardiac support, 20% renal, 5% neurological and 0.4% hepatic support. Covid-19 is clearly a complex clinical syndrome and not a straightforward viral pneumonia. Moreover, it is one where there is an ever present risk of iatrogenic damage, given the known hazards of prolonged ventilation and the need to keep pace with the rapidly increasing understanding of the pathological processes involved and the opportunities to intervene.
Clinicians managing covid-19 should have an awareness of this multi-system impact and actively seek evidence of organ involvement outside of the respiratory tract. Early involvement of the myocardium may be an important prognostic factor, with increasing trajectory of troponin associated with a fivefold risk in the need for ventilation and in mortality suggesting early monitoring would be helpful. Early proactive management to support renal function could improve later survival in ICU and aggressive thrombo-prophylaxis may reduce pulmonary emboli, while understanding the extent of organ involvement will guide escalation management decision making. These clinically driven interventions in medical care require urgent, real time evaluation of effectiveness.
It is too early to know what the long-term consequences of covid-19 will be. They are likely to include the effects of post intensive care syndrome. More than a quarter of patients suffer cognitive impairment with variable duration, psychological distress and physical detriment. There will in addition be sequelae of covid-19 infection itself as yet undefined. The experience of survivors of Severe Acute Respiratory Syndrome (SARS) points to a risk of sustained reductions in lung and exercise capacity and high levels of psychological distress. We must now urgently report studies of the follow up of discharged survivors of this multi-system disease to inform the provision of rehabilitation services and to better equip our primary care and community teams to manage the challenge of  patients with multi-system impairment discharged to home.

Unfortunately, this "weird" complication is also the deadliest as experts believe the SARS-CoV-2 virus is capable of creating tiny blood clots that can block arteries and even reach the brain.

According to a report by BGR, experts found out that the virus can cause blood coagulation and could create tiny blood clots that form in the lungs first--and now, medical doctors finally understand how it happens.

In a study penned by doctors from the St. James's Hospital, Irish Center for Vascular Biology, Royal College of Surgeons in Ireland, the researchers found that abnormal blood clotting is one of the causes of death of many coronavirus patients that were severely infected with the virus.
They have also found out that these tiny clots start in the lungs following the infection and that the higher the clotting, the worse the prognosis.
Most cases with worse prognosis also tend to require intensive care.
The study, which was published in the British Journal of Haematology, has already been peer-reviewed.

Reason Behind High Levels of Mortality

"Our novel findings demonstrate that COVID-19 is associated with a unique type of blood clotting disorder that is primarily focussed within the lungs and which undoubtedly contributes to the high levels of mortality being seen in patients with COVID-19," said Professor James O'Donnell, the director of the Irish Centre for Vascular Biology, RCSI.
The disorder is known as pulmonary intravascular coagulopathy (PIC).
It is different from disseminated intravascular coagulopathy (DIC) that is observed in various other medical conditions.
The disorder might be responsible for the blood clots reaching the heart or the brain of the patients, which could then lead to heart attacks or stroke that could be deadly to those with severe complications.

 PIC May Also Cause Oxygenation Issues

In addition, Professor O'Donnell said that in addition to pneumonia that affects the small air sacs within the lungs, they have also found hundreds of the tiny clots in the lungs of severely ill coronavirus patients--something they haven't seen in other respiratory infections.
The disorder could explain why "blood oxygen levels fall dramatically in severe COVID-19 infection," leading to the need for ventilator and oxygenation therapy.
As of now, the team will require further research to know whether blood-thinning medications could help high-risk patients against the formation of these tiny clots and help save their lives.
The team would also have to continue researching the complication in order to understand how the blood clots form in the lungs and find a way to develop more effective treatments for coronavirus patients.
Their study also found out that both race and ethnicity "have major effects upon thrombotic risk."
Caucasians and African-American patients are more likely to develop blood clotting disorder compared to Chinese patients.
The study says Chinese patients are four times less likely to develop them compared to Caucasian patients, while African-American patients have a significantly higher risk.

People who need the hospital are delaying treatment for fear of COVID-19 infection, doctors say

NEW ORLEANS Area hospitals and New Orleans EMS are reporting fewer cases of illness like heart attacks and strokes during the COVID-19 pandemic, leaving health officials to believe people might be avoiding emergency rooms because they fear they might be infected.
I would say we are about 40% down in our volume currently from what wed normally be seeing at this point of the year," said Roland Waguespack, medical director at East Jefferson General Emergency Department.

He believes the drop in numbers of patients with stroke and heart attacks doesn't mean these illness aren't happening.
"In south Louisiana and New Orleans -- those medical conditions don't disappear, Waguespack said. They are happening and what we are concerned about is that people are afraid that we can't keep them safe in our emergency rooms and hospitals and that they are staying away from us."
In addition to East Jefferson, Tulane Medical Center Emergency Room says they have seen evidence that people are unnecessarily delaying care and believe its negatively impacting their health.
University Medical Center also confirmed they are seeing lower numbers of patients coming in for conditions that they are used to, including heart attacks and stroke. That is a concern, according to a spokesperson from the hospital, but maintain the emergency department is ready to safely treat patients with any illness.
According to EMS Director Dr. Emily Nichols, EMS saw a 25% drop in people using 9-1-1 with these conditions. Nichols expects a bigger drop in April and is worried for those who wait to get care.
"Both stroke and heart attack are critical medical emergencies, Nichols said during a press conference at EMS headquarters Wednesday. They require early recognition and if they are not treated they could be fatal."
In the last seven weeks, medical officials at Tulane Medical Center emergency room report that they have still seen about the same number of stroke patients; however, staff hasnt been able to use the time sensitive clot busting drug, tPa.
Typically ,tPa needs to be administered within a few hours of the onset of stroke symptoms. We were able to administer tPa just once (this week), said Sarah Balyeat, communications coordinator at Tulane Medical Center, in an email. All other stroke patients have arrived too late for the clot busting drug to be effective.

Area hospitals are fully staffed to treat these critical patients and are taking precautions like screening for COVID-19 and separating positive patients from others. Doctors are also using appropriate PPE, according to Sarah Babcock with the New Orleans Health Dept.
"We have more room now than we did before COVID-19 -- our hospitals -- I can speak to my hospital -- East Jefferson has had the resources we'd needed and have been able to take care of patients of all illnesses during this time," Waguespack said.
Symptoms of a stroke include sudden weakness, numbness or confusion. Symptoms of a heart attack include chest pain, shortness of breath or nausea.
While sometimes these symptoms are severe, doctors say, other times they are more mild. Make no mistake though, the longer you wait, especially for a stroke, the worse it could be, according to Balyeat.
The longer you wait to get to the hospital the more likely you are to have long term deficiencies, Balyeat said.

In Hungary, a new law giving the prime minister wide-ranging emergency powers includes a provision prohibiting publication of "false information" about the pandemic, with violators facing up to five years in prison. Journalists there 
told the German public news outlet Deutsche Welle
 that the law has generated uncertainty over whether they could be prosecuted for reporting true information, could harm independent news outlets' revenue streams, and makes the already difficult job of obtaining information even more so.
In China, the government maintains that controlling information is necessary to combat the disease. Chen Qiushi, a citizen journalist who traveled in January to Wuhan, where the virus was discovered, and who posted videos on YouTube reporting that hospitals were overwhelmed, 
has not been heard from
 since Feb. 6.
This "Covid-19 crackdown" as Joel Simon, the executive director of the Committee to Protect Journalists, 
has called it
 is unfolding as we prepare to mark 
World Press Freedom Day
 on May 3. This annual event serves as a reminder to respect the press, defend journalists and news organizations from attacks on their independence, and remember journalists who have lost their lives while doing the heroic work of keeping the public informed under difficult circumstances. The enormous challenges and high stakes of this pandemic have created an urgent need for independent, timely and credible information. Moreover, the daily onslaught of misinformation has made this literally a matter of life and death. Yet many repressive regimes and autocratic leaders are seeking to restrict the press even as journalists are working to hold governments accountable for their response to this pandemic and to report on the growing public health crisis and the concomitant financial toll.
Particularly in China, we've seen how self-defeating press restrictions can be in combating the spread of Covid-19. On March 24, Reporters Without Borders (also known as Reporters sans frontières, or RSF), a Paris-based nongovernmental organization that advocates for freedom of information, 
wrote
 that "without the control and censorship imposed by the authorities, the Chinese media would have informed the public much earlier of the severity of the coronavirus epidemic, sparing thousands of lives and perhaps avoiding the current pandemic."
RSF has created 
Tracker 19
, a tool designed to evaluate Covid-19's impact on journalism (the name refers both to the disease and to Article 19 of the 
Universal Declaration of Human Rights
). A map on the Tracker 19 website shows documented instances across five continents of state censorship, deliberate disinformation and other efforts that obstruct the public's right to information. RSF has compiled a list of concerning developments in more than 40 countries.
The abuse of press freedom is hardly limited to authoritarian nations. A 
new law
 in South Africa which has guaranteed press freedom since its 1996 constitution and which enjoys a tradition of independent journalism has made it a crime to publish "disinformation" about the pandemic.
Developments in India, the world's largest democracy, are even more troubling. Prime Minister Narendra Modi's government asked the country's Supreme Court to require news outlets to obtain state permission before publishing any coronavirus-related content essentially empowering the government to censor coverage. While the court denied the request, it ordered news organizations to "publish the official version" of coronavirus developments,

Responsible reporting from journalists who are free to call out mismanagement of the pandemic and deception by government officials has never been more vital. In February, the World Health Organization 
termed
 the swell of rapidly evolving information about the Covid-19 outbreak including a flood of misinformation an "infodemic." On April 14, United Nations Secretary-General António Guterres referred to this "dangerous epidemic of misinformation" as 
"a poison that is putting even more lives at risk."
Against this backdrop of fear, anxiety and uncertainty, World Press Freedom Day takes on a deeper meaning. The growing limits on press freedoms not only make it more difficult for journalists to help keep the public informed, but also undercut the ability of journalists to help keep the public safe.
At a time when their work is most needed, journalists worldwide are risking not only their freedom but their lives on the front lines to cover this devastating pandemic. They deserve our appreciation and need our support more than ever.

In 1976, the artist Saul Steinberg drew a cover for The New Yorker View of the World from Ninth Avenue that became an instant classic. You know the one: Manhattan heavily in the foreground, the Hudson River, a brownish strip called Jersey, the rest of the America vaguely in the distance.
It could almost be a map of the coronavirus epidemic in the United States.
Even now, it is stunning to contemplate the extent to which the countrys Covid-19 crisis is a New York crisis by which I mean the city itself along with its wider metropolitan area.
As of Friday, there have been more Covid-19 fatalities on Long Islands Nassau County (population 1.4 million) than in all of California (population 40 million). There have been more fatalities in Westchester County (989) than in Texas (611). The number of Covid deaths per 100,000 residents in New York City (132) is more than 16 times what it is in Americas next largest city, Los Angeles (8). If New York City proper were a state, it would have suffered more fatalities than 41 other states combined.
It isnt hard to guess why. New York has, by far, the highest population density in the U.S. among cities of 100,000 or more. Commuters crowd trains, office workers crowd elevators, diners crowd restaurants. No other American city has the same kind of jammed pedestrian life as New York Times Square alone gets 40 million visitors a year or as many residents packed into high-rises. The city even has a neighborhood called Corona, which, it turns out, has among the highest rates of coronavirus infections.
Consider a thought experiment in which metropolitan New York werent just its own state, but its own country. What would the crisis for what remained of America look like, then? In this slightly smaller nation of a little more than 300 million people, the death toll would amount to about 7.5 per 100,000, slightly above Germanys levels.
No wonder so much of America has dwindling sympathy with the idea of prolonging lockdown conditions much further. The curves are flattening; hospital systems havent come close to being overwhelmed; Americans have adapted to new etiquettes of social distancing. Many of the worst Covid outbreaks outside New York (such as at Chicagos Cook County Jail or the Smithfield Foods processing plant in Sioux Falls, S.D.) have specific causes that can be addressed without population-wide lockdowns.
Yet Americans are being told they must still play by New York rules with all the hardships they entail despite having neither New Yorks living conditions nor New Yorks health outcomes. This is bad medicine, misguided public policy, and horrible politics.
On Friday, I spoke with Tomislav Mihaljevic, C.E.O. of the widely admired Cleveland Clinic, and an advocate of the need to use tailored and discriminating solutions that also recognize regional differences. At the moment, he says, Were using the methodology from the 14th century to combat the biggest pandemic of the 21st century. It cant go on.
Dr. Mihaljevic acknowledges the necessity of the lockdowns to contain the virus, along with the urgent need for ramped-up testing and ongoing monitoring. But, he adds, we cannot hold our breath forever. The U.S. will not soon be able to test 330 million people. Effective therapies or vaccines may be long in coming. Covid-19 will be a disease we have to learn to live with.
That means accepting that the immediate goal of public policy cannot be to eliminate the risk of Covid-19. It is to mitigate, manage and frame expectations for it while not losing sight of other priorities. In Ohio Dr. Mihaljevic says that Covid patients take up just 2 percent of hospital capacity, and the curve of new infections has been flat for more than two weeks. Yet there has been a dramatic decline in people seeking care for heart attacks, strokes, or new cancers, presumably out of fear of going to hospital.
The public conversation needs to be about the value of human life in its totality, Dr. Mihaljevic says. That includes fewer restrictions on activity for people at the low end of the risk spectrum, while taking additional care of those on the high end.
Right now, theres a lot of commentary coming from talking heads (many of them in New York) about the danger of lifting lockdowns in places like Tennessee. Perhaps the commentary needs to move in the opposite direction. Tennesseeans are within their rights to return to a semblance of normal life while demanding longer restrictions on New Yorkers.
I write this from New York, so its an argument against my personal interest. But I dont see why people living in a Nashville suburb should not be allowed to return to their jobs because people like me choose to live, travel and work in urban sardine cans.
Gina Raimondo, the Rhode Island governor, was on to something when, a few weeks ago, she wanted to quarantine drivers arriving from New York. The rest of America needs to get back to life. We New Yorkers prefer our own company, anyway.

Our crippled economys future hangs on the effectiveness of trillions in hastily dispensed federal aid and trillions more in franticly printed money. But as the Covid-19 virus begins to relent, demands for federal cash have not.
The next bailout will likely be directed toward fiscally-mismanaged blue states like Connecticut, which were barely making it before huge chunks of their tax revenues were severed by the shutdown. Democrat administrations all over the country will seize the opportunity to force more fiscally astute states to pay for years of excessive union pay raises, employee retirement largesse and liberal welfare policies, all intended to buy votes, which trapped their states into irretrievable fiscal holes.
Excessive money printing usually leads to high inflation and high interest rates. That happened during President Jimmy Carters administration with only a tiny fraction of the fiscal provocation happening today. The staggering double-digit inflation and double-digit interest rates of those years unfortunately werent even mitigated by the boom times that usually cause of such travails.
Instead we had stagflation, inflation caused by too much money flooding an unproductive economy with high unemployment.
Now millions of working Americans have been laid off by government-forced business shutdowns and given long-term state and federal unemployment payments that often far exceed what they would earn if still employed. That has laid the foundation for another bout of stagflation unproductive workers, flush with unexpected cash, bidding up commodity prices in an under-producing economy.
The idea was to keep workers ready to step back into their old jobs as work became available, but a local restaurant owner is already confronting what conservatives warned would happen. With business picking up, he is trying to bring back some kitchen staff. None of them are returning his calls, he said, probably because they would be obliged to return to work and sacrifice the extra government-provided income.
If the economy finally does begin to pick up steam it all could be cut short by a resurgent Covid-19 assault next fall, especially if combined with a hard flu season. With FDA vaccine approval impossible to get by then, government policy consistency would require more lockdowns. That would be a knockout blow to already staggered small businesses.
Sweden may be proving it didnt have to be this way. That country was the only one in Europe that didnt adopt the lockdown strategy that gripped most of the globe. Its public schools, restaurants, bars, hair salons and other businesses remained open, although events drawing crowds of over 50 were prohibited and Swedes were asked to politely keep their distance from unrelated fellow citizens. Turns out Swedes like to do that anyway. Their Covid-19 death rate is higher than other locked down Scandinavian countries but much lower than the aggressively locked down countries to its south, including the UK, Italy and Spain. And although Swedens export-driven economy has been hit by the global slowdown, it is doing relatively well.
Best of all, close to a third of Swedens population may already have antibodies from exposure to the virus, meaning it is almost halfway to the 65 percent herd immunity level that can virtually eliminate the threat of a future pandemic. We may have simply put off that reckoning and destroyed our economy in the process.

Hispanic people in King County have died from COVID-19 at a rate nearly two-and-a-half times higher than that of white people, according to a new study of data released Friday showing the novel coronavirus is disproportionately affecting people of color.
Hispanic, Black and Native Hawaiian/Pacific Islander groups also have been infected and hospitalized at significantly higher rates in King County than white people, according to the analysis conducted by Public Health Seattle & King County.
The new figures mirror national disparities in COVID-19 cases as well as a higher prevalence of other diseases for communities of color in general, public-health officials said. The analysis also underscores the effects of long-standing inequities in health care, social services, housing and other socio-economic challenges, they said.
No one should be surprised that were seeing these disparities in COVID-19 disease, said Dr. Jeff Duchin, local health officer for Seattle and King County. Its been an ongoing national tragedy and shame that we have communities of color throughout our country suffering disproportionate adverse health impacts from a wide variety of health conditions.
Reasons for the disparities are still being examined, but officials said there are probably multiple contributors. Some likely causes include unequal access to diagnostic testing, fear among immigrants and refugees to seek medical treatment, and the fact that people of color are statistically more likely to work service-oriented jobs that leave them more vulnerable to virus exposure, they said.
More communities of color, for example, live in multi-generational housing that makes it difficult to physically distance and can lead to more infections, said Matias Valenzuela, equity director for Public Health Seattle & King County. Who are the essential workers, who are the people who are having to go out and work during these challenging times? The numbers are disproportionately people of color.

The new insight into the coronavirus uneven impact in King County comes into focus as people of color represent an increasing percentage of the countys COVID-19 cases.
That trend appears to reflect how King Countys outbreak unfolded, Duchin said, with most of the first cases detected among white people connected to local nursing homes. In the weeks since, more cases attributed to widespread community transmission have piled up.
As the outbreak evolves, the percent of cases by race and ethnicity appears to be shifting, and the disparities are becoming increasingly apparent, Duchin said.

More on the coronavirus outbreak

The demographic analysis, which was published Friday on the health departments website and will be updated, is based on totals reported in King County as of Thursday: 6,428 infections and 441 deaths. The study examines per-capita calculations for each racial and ethnic group, using the number of cases per 100,000 residents.
Washington does not currently require racial and ethnic data to be reported to public-health jurisdictions collecting information for infectious disease case reports. So, to conduct this analysis, public-health officials had to draw information from patient interviews, additional medical records and other sources.
The other thing thats enabled us to do this analysis is just increasing numbers of cases, Duchin said. We have a large enough database now that we can do these analyses with supplemental matching of race and ethnicity data.

The study shows that demographics of the disease in King County have changed drastically over the past six weeks.
On March 8, white people made up about 72% of the countys cases, with Asians at 16%, Hispanic people at 6%, Black people 5% and Native Hawaiian/Pacific Islanders 1%.
As of April 26, only half the countys cases were white people and 13% were Asians, while rates have spiked for the other groups. For Hispanic people, the proportion nearly quadrupled, to 23%. For Black people and Native Hawaiian/Pacific Islanders, it doubled to 10% and 2%, respectively.
By comparison, 58.7% of King Countys population is white, 17.2% is Asian, 10% is Hispanic or Latinx, 6.4% is black and 0.8% is Native Hawaiian/Pacific Islander.
The disparity is particularly stark in Hispanic and Latinx communities. Hispanic people in King County are infected and hospitalized at four times the rate of white people.
Weve had 441 deaths in King County, and although 297 occurred among whites and 34 among Hispanics, the rate of death is higher among Hispanics, Duchin said. Its about two-and-a half times higher compared to whites.

Native Hawaiians/Pacific Islanders were infected at about four-and-a-half times the rate per capita than white people. Black people had about twice the rate of cases per capita as white people.
Case rates also are significantly higher among American Indian and Native Alaskan populations, Duchin said, but due to the small sample size from those groups so far only 29 cases in all the data is not statistically significant. Asians showed a similar rate of reported cases to that of white people, he said.
The analysis includes an overlay of case data on census tract maps to generally show that higher case rates exist in South Seattle and South King County, where more people of color live. Limited data by occupation and by neighborhood makes it challenging to dig deeper at this point, but officials plan to publicize more data and maps in the days ahead, Duchin said.
Coronavirus disparities are not exclusive to King County. A recent CDC study of COVID-19 data drawn from 14 states found Black people were over-represented among 580 hospitalized patients, and a New York City Health study identified death rates substantially higher among Black and Hispanic people compared to white people and Asians.
Deep-seated societal problems from institutional racism and language barriers to inequitable access to housing, education, work and health care are among the ongoing and complex contributors that exacerbate disproportionality in disease cases, Valenzuela said.
An estimated 70,000 to 80,000 undocumented people who dont qualify for federal health coverage, stimulus funding and other services live in King County, he said.

This is not an easy issue, Valenzuela said. We know that there are root causes and underlying issues that really drive the outcomes that we see across all diseases.
To address some of the drivers of uneven case rates during the pandemic, public-health officials are meeting regularly with a community advisory group, translating public health materials in various languages and supporting state efforts to test more people and collect more data, officials said.
But we need to do more, Valenzuela said.
Across all racial and ethnic populations, reported cases of COVID-19 remain too high to ease up on social-distancing restrictions, Duchin said. Gov. Jay Inslee on Friday afternoon extended the states stay-home order through May 31, saying that public-health data such as the number of daily confirmed cases, and the transmission rate of the virus in King County has not yet been favorable enough.
Duchin said its critical to ramp up testing for everyone so we know who has the virus and can prevent more people from being infected.
We need to get down to a much lower number of cases before relaxing, he said. In order to do that, we need a much better understanding of how people are becoming infected.
