It is difficult to determine this exactly, of course.  Now we started to see the tip of the iceberg. This figure: 10-20% health care workers of all cases, probably indicates, that the virus was circulating for some time, and the un-diagnosed infectious people were infecting the health care workers, who in turn were unknowingly infecting the new cohorts of their patients, who saw them for the other reasons.  It also looks like there are at least two, maybe three or more distinct clinical entities of this
