Washington, Apr 18 (IANS): Genetic variability in the human immune system may affect susceptibility to, and severity of infection by SARS-CoV-2, the virus that causes the coronavirus disease (COVID-19).
The findings published in the Journal of Virology may help shed new light into the puzzling question of why the disease hits some people harder than others.
Certain immune system genes, called human leukocyte antigen genes that are involved in recognising pathogens, vary from person to person.
These variations can influence how well the immune system recognises a given pathogen.
Poor recognition of SARS-CoV-2 could cause a person to be more vulnerable to the virus, said the study.
"In particular, understanding how variation in HLA (a component of the immune system containing multiple genes) may affect the course of COVID-19 could help identify individuals at higher risk from the disease," according to the authors of the new study.
The authors showed that individual HLA, haplotype, and full genotype variability likely influence the capacity to respond to SARS-CoV-2 infection, and noted that certain alleles in particular could be associated with more severe infection, as has previously been shown with SARS-CoV.
"This is the first study to report global distributions of HLA types and haplotypes with potential epidemiological ramifications in the setting of the current pandemic," said the authors from Oregon Health & Science University, Portland, and the Portland VA Research Foundation in the US.
"HLA typing can be fast and inexpensive," the authors wrote.
"Pairing HLA typing with COVID-19 testing where feasible could improve assessment of viral severity in the population. Following the development of a vaccine against SARS-CoV-2, the virus that causes COVID-19, individuals with high-risk HLA types could be prioritized for vaccination."

Last week, as it emerged that the first ten doctors to die of Covid-19 were black, Asian, or from other ethnic minorities, health chiefs called for an investigation into why these groups were apparently worse affected by the outbreak.
Figures suggest 28 per cent of the most seriously ill patients needing hospital treatment for coronavirus infection are either black or Asian – despite making up around ten per cent of the UK population.
There have also been concerns over what appears to be a disproportionate number of deaths among black and Asian bus drivers.
A similar trend has been noted in the US, where 70 per cent of the deaths in Louisiana have been among black Americans, more than double the background population.
But experts remain unclear about what might be driving the anomaly. Dr Riyaz Patel, Associate Professor of Cardiology, at University College London, said: ‘This could be because of socioeconomic factors, with BME [black and minority ethnic] patients more likely to live in densely populated areas, multi-generational families in the same household and having more public facing jobs, all of which makes the likelihood and duration of exposure to the virus more likely.
‘Another explanation is that the virus has so far hit densely populated areas like London first, where BME populations are high. Nonetheless, there could be biological reasons for the difference, which we can speculate on.
‘One thing very visible to us in London ITUs now is how diabetes, high blood pressure and possibly being a little overweight, seem to be such potent risk factors for having severe lung illness. All of these risk factors are more common in Black and Asian patients, so there could be a link here to explorate.’

There could also be a genetic explanation, although not one which has been investigated specifically for coronavirus.
Immunologist Professor Philip Goulder, at the University of Oxford, said some immune cell molecules, called HLA, can vary between populations.
HLA allows tiny fragments of proteins from viruses like the coronavirus to be recognised by the immune system.
In West Africa, where there are high rates of malaria, much of the local population has an HLA molecule called B53, which protects against death from the disease. But this is rarely found in the UK population. Some forms of HLA are also thought to protect against HIV, while others make the disease more serious.
Prof Goulder said: ‘Certain classes of molecule are protective, and some are associated with much more rapid disease. It’s almost certainly going to apply to coronavirus.’

Can genetics explain the degrees of misery inflicted by the coronavirus?

One of the most terrifying aspects of the COVID-19 pandemic is that we don’t know what makes one person die, another suffer for weeks, another have just a cough and fatigue, and yet another have no symptoms at all. Even the experts are flummoxed.
“I’ve been puzzled from the beginning by the sharp dichotomy of who gets sick. At first it was mostly older people with chronic disease, and then a young person with low risk would show up. It can be devastating and rapid in one individual but mild in another,” said Anthony Fauci, MD, director of the National Institute of Allergy and Infectious Disease on a media webinar.
What lies behind susceptibility to COVID-19? Gender? Genetics? Geography? Behavior? Immunity? All of these factors may be at play, and they overlap.
Comedian Bill Maher blames poor immunity on eating too much sugar, and a thriving industry pitches immune-boosters, but much of the strength or weakness of an individual’s immune response arises from specific combinations of inherited gene variants. That’s my take as a geneticist, and Dr. Fauci’s. “Perhaps genetics and the immune response play a role in why one person has a mild response, yet another rapidly deteriorates into viral pneumonia and respiratory failure,” he said.

Learning as we go

During the first weeks of the pandemic, the observation that many victims were either older, had certain chronic medical conditions, or both, fed a sense of denial so widespread that young people flocked to Spring Break beaches as older folks boarded cruise ships in Florida as recently as early March. And then the exceptions began to appear among the young people.
While clinicians on the front lines – everywhere – are saving as many lives as possible, researchers are racing to identify factors that the most vulnerable, and the most mildly affected, share, especially the asymptomatic carriers. And as the numbers continue to climb and more familiar possible risk factors are minimized or dismissed – age, location, lifestyle habits – genetics is emerging as an explanation for why otherwise young, strong, healthy people can die from COVID-19.
Following are possible genetic explanations for why some people become sicker than others. These are hypotheses, the language of science: ideas eventually fleshed out with observations and data. “Proof” is part of mathematics; in science, conclusions can change with new data. The public is getting a crash course in the scientific method.

Twice as many males as females: the role of the X

The most obvious genetic risk factor in susceptibility to COVID-19 is being male. The details of disease demographics change daily, but males are about twice as likely to die if they’re infected as are women: 4.7% versus 2.8%.
At first people blamed the sex disparity on stereotypes, like the riskier habits of many a male compared to females. But the sex difference comes down to chromosomes.

In humans, a gene, SRY, on the Y chromosome determines sex. Males have one X and a puny Y; females have two Xs. Fortunately, nature takes care of this fundamental inequality of the sexes, which I detailed here and in every biology textbook I’ve ever written.
To compensate for the X deficit of the male, one X in every cell of a female is silenced beneath a coating of methyl groups, an epigenetic change. But which X is silenced differs, more or less at random. In a liver cell, the turned off X might be the one that the woman inherited from her father; in a skin cell, the silenced X might be the one inherited from her mother.
The immune system seems to benefit from the female’s patchwork expression of her X-linked genes, with a dual response. Gene variants on one X may recognize viruses, while gene variants on the other X may have a different role, such as killing virally-infected cells.
Women also make more antibodies against several viral pathogens. But some of us pay the price for our robust immune response with the autoimmune disorders that we are more likely to get.

ABO blood type

People with type O blood may be at lower risk, and with type A blood at higher risk, of getting sick from SARS-CoV-2, according to results of a recent population-based study. But the idea of type O blood protecting against viral infections goes back years.
We have three dozen blood types. They’re inherited through genes that encode proteins that dot red blood cell (RBC) surfaces, most serving as docks for sugars that are attached one piece at a time. The RBCs of people with type O blood do not have an extra bit of a sugar that determines the other ABO types: A, B, or AB.
The unadorned RBCs of people with type O blood, like me, are less likely to latch onto norovirus (which explains why I rarely throw up), hepatitis B virus, and HIV.

People with blood types B and O make antibodies that block the binding of the SARS virus’s spikes to ACE2 receptors on human cells growing in culture. That’s what researchers from several institutions in China have found in the new study. They compared the blood types of 2,173 patients with COVID-19 from three hospitals in Wuhan and Shenzhen to the distribution of blood types in the general population in each area.
People with type A blood were at higher risk than people with type 0 blood for both infection and severity of the illness.
In the general population 31% of the people are type A, 24% are type B, 9% are type AB, and 34% are type O. But among infected individuals, type A is up to 38%, type B up to 26%, AB at 10%, and type O way down to 25%.
The researchers conclude that the findings demonstrate “that the ABO blood type is a biomarker for differential susceptibility of COVID-19.” I think that’s a bit strong for a trend, considering the exceptions. But the researchers suggest that their findings, if validated for more people, can be used to prioritize limited PPE resources and implement more vigilant surveillance and aggressive treatment for people with blood type A.

A few good genes

Immunity and genetics are intimately intertwined. Links between mutations – both harmful and helpful – and immunity to infectious diseases are well known.
Mutations in single genes lie behind several types of severe combined immune deficiencies (SCIDs), like “bubble boy disease.” Sets of human leukocyte antigen gene variants (HLA types) have long been associated with increased risk of autoimmune conditions such as celiac disease, type 1 diabetes, and rheumatoid arthritis, and were for many years the basis of tissue typing for transplants.

In HIV/AIDS, two specific mutations in the CCR5 gene remove a chunk of a co-receptor protein to which the virus must bind to enter a human cell. The mutation has inspired treatment strategies, including drugs, stem cell transplants, and using CRISPR to recreate the CCR5 deletion mutation by editing out part of the gene.
Might variants of the gene that encodes ACE2, the protein receptor for the novel coronavirus, protect people in the way that a CCR5 mutation blocks entry of HIV? The search is on.
Another clue to possible genetic protection against the novel coronavirus may come from the SARS experience from years ago and parasitic worm diseases in Africa. (This hypothesis I came up with on my own so I’m prepared to be shouted down.)
In a human body, the SARS virus disrupts the balance of helper T cells, boosting the number of cells that fight parasitic worms (the Th2 response) while depleting the cells that protect against bacteria and viruses (the Th1 response). The resulting “Th2 immune bias,” in SARS as well as in COVID-19, unleashes the inflammatory “cytokine storm” that can progress to respiratory failure, shock, and organ failure.
In subSarahan Africa alone, a billion people have intestinal infections of parasitic worms, the most common of which is schistosomiasis. It’s also called “snail fever” because the worms are released into fresh water from snails and burrow into people’s feet when they wade in the water.
The worms mate inside our blood vessels, releasing eggs that leave in urine and feces into the water supply. Remaining eggs can inflame the intestines and bladder. The infection begins with a rash or itch, and causes fever, cough, and muscle aches in a month or two. A drug treatment is highly effective.

Genetics determines susceptibility, or resistance to, schistosomiasis. And that’s what got me thinking about COVID-19.
People who resist the flatworm infection have variants of eight genes that ignite a powerful Th2 immune response that pours out a brew of specific interferons and interleukins. Could the “Th2 immune bias” of the novel coronavirus SARS-CoV-2 not be as devastating to people who already have the bias, to resist schistosomiasis? If so, then places in Africa where many people are immune to schistosomiasis might have fewer cases of COVID-19.
So far parts of Africa have reported low incidence of the new disease. On April 7, the World Health Organization reported approximately 10,000 cases in all of Africa. That’s similar to the number of deaths in New York City, although Africa could be on track for the exponential growth seen elsewhere. But if the lower number in Africa persists, then maybe those eight genes are protecting  people. Adding to the evidence is that the 8-gene set varies more between West Africans and Europeans than do other sets of genes.
Like the ABO blood type study, if the 8-gene signature that protects against schistosomiasis protects against COVID-19, then the signature should be overrepresented among those exposed to the virus who do not get very sick, and underrepresented among those who do. However, it’s possible that Africa is just behind the rest of the world in reporting COVID-19 cases. So, a thought experiment for now.

Considering genetics

Before researchers zero in on a highly predictive genetic signature of COVID-19 risk, we can think about how the information would best be used:

  • To triage patients with early symptoms, to predict who will become sickest.
  • To use resources in an informed way.
  • To inspire development of new drugs and investigating repurposing of drugs that have mechanisms of action that suggest that they might destroy SARS-CoV-2.
I hope that discovery of a genetic basis for COVID-19 vulnerability or resistance will not inspire discrimination – unfortunately, genetic information has had a legacy of misuse.

Who Gets Sickest From COVID-19?

It’s a recurring theme of the COVID-19 crisis: The virus sweeps its way through a community, and, despite being exposed to it in the same place at around the same time, people develop vastly different symptoms. Some barely feel anything — a scratchy throat, if that — while others spend weeks in the ICU with ravaged lungs, unable to breathe on their own.
These scattershot outcomes stem from wide variation in how our bodies respond to the virus. To mount the strongest defense, says Boston Children’s Hospital immunologist Hani Harb, the immune system must maintain a delicate equilibrium. “We need the balance of the force attacking the virus, and at the same time, a counterforce to say, ‘That's enough.’ ”
If anything interferes with optimal immune function, such as an errant gene, a lifestyle habit or a chronic condition, COVID-19 can wreak more havoc in the body than it otherwise would.

Genetic vs. Acquired

Among the main drivers of our innate immune response is a set of genes known as the HLA (human leukocyte antigen) complex. These genes code for proteins in the surfaces of cells that display signals to the immune system, and differences in these signals make the immune system react differently to invading pathogens. In this way, HLA genes affect our susceptibility to a host of illnesses, including viral ones. Scientists at the Chinese University of Hong Kong and elsewhere have found a particular HLA gene variant tied to high rates of severe symptoms of SARS, a coronavirus related to COVID-19.
Because COVID-19 only recently appeared in humans, we don’t know exactly which genetic quirks might make us more susceptible to it. Scientists are now investigating whether specific HLA genes give some people higher or lower degrees of protection against the virus. If they can identify such genes, companies could go on to create detection kits that would give test-takers an idea of how prone they may be to the illness.
But genes are only the first component of our COVID-19 response. So-called trained immunity is just as key in bolstering viral resistance, says Brianne Barker, an immunologist at Drew University. Our immune systems are far from immutable — they’re actually incredibly fluid, learning from the invaders they encounter and adapting their defenses accordingly. Some researchers speculate that if you’ve gotten other coronaviruses in the past, your immunity to COVID-19 might be somewhat higher, though by no means airtight.
Your lifestyle habits also shape your body’s reaction to the disease. COVID-19 uses a cell surface receptor called ACE2 to enter the cells that line your respiratory tract. New research shows that in smokers, these receptors are more prevalent, creating more potential access routes for the virus. “If you smoke,” Harb says, “the virus will be able to enter more cells in higher numbers.”

Taming the Storm

One of the biggest determinants of how sick you’ll get from COVID-19 is whether your body unleashes what’s known as a cytokine storm against the virus. After COVID-19 forces its way into cells, immune signaling proteins called cytokines act like warning sirens, bringing the body’s cellular attack force to the scene. Cytokines are “normally made in any immune response to recruit more immune cells or induce tissue repair,” Barker says.
Problems arise, however, when the body starts churning out too many cytokines, calling up a platoon of attacker cells equivalent to a D-Day invasion force. These out-of-control cytokines and marauding cells ravage healthy tissue, creating extensive lung damage that defies the body’s normal repair efforts.
In most people with healthy immune systems, control cells called regulatory T cells help turn down the volume of attacker cells. These T cells’ purpose “is to keep everything quiet — see that all the [other] cells are functioning properly without being overstimulated,” Harb says.
But in some older people, or in those who have underlying immune deficits from chronic conditions, regulatory T cells do not function normally. When these people get COVID-19, cytokine storms may cause excessive inflammation in the lungs and whip up new torrents of attacker cells, leading to life-threatening disease. One Chinese study found that COVID-19 patients with severe illness had lower levels of regulatory T cells in their bloodstream. Children, on the other hand, may be less prone to disabling symptoms in part because their immune systems are better regulated and because they have fewer underlying conditions.
There might be ways to tame deadly cytokine storms that strike the vulnerable, Barker says. One cytokine in particular, called interleukin 6 (IL-6), can cause highly damaging inflammation, so researchers have started a clinical trial of an arthritis drug called tocilizumab — which blocks the action of IL-6 — to see if it can curb symptoms of COVID-19.
A safe and effective COVID-19 vaccine will ultimately be the best way to even the immune playing field for everyone. In the meantime, scientists will continue studying the broad range of immune responses to the virus — a still-muddled landscape in which rules seldom apply across the board. “I have seen patients over 70 who have the disease and they are fine, and there are many young, healthy people who are dying,” Harb says. “We have a lot to research. There is still a hidden component.”

DNA Could Hold Clues to Varying Severity of COVID-19

Among the many mysteries that remain about COVID-19, the disease caused by the new coronavirus, is why it hits some people harder than others. Millions of people have been infected, but many never get sick. Those who do can experience an ever-expanding array of symptoms, including loss of smell or taste, pink eye, digestive issues, fever, cough, and difficulty breathing. Although the elderly, those with pre-existing conditions such as heart disease, and men are most likely to suffer severe complications, hundreds of young and previously healthy people have died from the disease in the US alone.
In recent weeks, researchers have begun asking whether genetics could influence the severity of symptoms. 
So far, they know “basically nothing,” Wendy Chung, a clinical geneticist and physician at Columbia University, tells The Scientist. She is one of hundreds of scientists launching studies to interrogate the human genome for answers. Chung and her team are racing to “recycle” and bank nasal swabs and other clinical samples from COVID-19 patients across the New York-Presbyterian Hospital System, currently in the epicenter of the coronavirus pandemic.  
The researchers plan to extract the patients’ DNA and scan the genomes for tiny sequence variations associated with symptoms listed in their electronic health records. 
Prior research has uncovered gene variants that can alter a person’s chances of contracting an infectious disease. The most famous example is a mutation in the CCR5 gene, which offers protection against HIV. 
Other variants can affect what happens once the virus is inside the human body, leading to strikingly disparate outcomes from one person to the next, says Priya Duggal, a geneticist at Johns Hopkins University. Duggal has previously shown that variants in the human leukocyte antigen (HLA) genes, which influence the body’s immune response, may explain why some people spontaneously clear hepatitis C infection whereas others are left with chronic disease. 
Duggal says understanding how genetic background affects people’s responses to infection may give scientists proteins or pathways to target to boost the immune response with a vaccine. That’s the reason she recently expanded her lab’s research from HIV, hepatitis, and other pathogens to look at the SARS-CoV-2 coronavirus. She is planning a study of younger people who have been hospitalized after contracting the virus to see if there is a genetic basis for their more serious disease, though she hesitates to guess what might come up in her search. 
“I think we’re so bad at predicting [which genes matter]. We’ve been terrible in the past and I think a lot of it is because we don’t fully understand everything that’s going on in the immune system,” she says. One of the reasons COVID-19 can be so fatal to some people is because of their bodies elicit an overzealous immune response called a cytokine storm, which may originate in their DNA. A small study of patients who died from the 2009 H1N1 flu outbreak found that many carried mutations that triggered this self-destructive flood of cytokine molecules.
Results from studies on the genetics of COVID-19 susceptibility and severity are beginning to trickle in. One study suggests that variants in the HLA genes likely play a role. Others point to differences in the ABO blood type, as well as variants in the ACE2 gene, which codes for the protein SARS-CoV-2 latches onto to infect human cells. But the findings are all preliminary, and require follow-up with larger datasets.

Andrea Ganna, a biostatistician at the University of Helsinki, is leading a major effort to pool data from genomics projects around the globe. The COVID-19 Host Genetics Initiative includes 117 studies on its website, and more than 439 scientists on its Slack channel. Ganna says several big biobanks have agreed to share the DNA data they have been gathering since before the pandemic, including the Penn Medicine Biobank, which has 60,000 participants; FinnGen, which has collected DNA from 5 percent of Finland’s entire population; and the UK Biobank, one of the world’s largest with samples from 500,000 volunteers. 
“I’m quite confident when these big players come on board, we can grow our databases exponentially in the next month and that we will find something,” says Ganna, who is hoping to identify gene variants associated with COVID-19 outcomes. 
Earlier this month, the personal genomics company 23andMe announced it would be tapping its database of more than 10 million customers for clues. Joyce Tung, 23andMe’s vice president of research, says her group plans to roll out surveys to the more than 80 percent of customers who have consented to be part of research. They hope to get hundreds of thousands of volunteers to enroll in the study, which will ask questions about their social distancing measures, symptoms, and COVID-19 test results. 
Every new positive case of COVID-19 provides another valuable data point for genomic studies. But if the pandemic wanes significantly, it is possible some studies may never be completed. “In some ways we’re playing a weird game, because the best scenario would be if we can’t get enough data because there weren’t enough cases,” says Tung.    
For now, the rapidly moving nature of the pandemic, and the infection itself, poses unique challenges. For their study, Chung’s and her colleagues have already identified thousands of positive cases, which they are consenting remotely. Several times, coordinators have called potential study subjects after they’ve been discharged, only to find out they’ve died. 
Chung, who pivoted her own work on rare diseases to contribute to COVID-19 research, says she feels a tremendous sense of urgency. “We think about what we can learn in two weeks, not what we can learn in two years,” says Chung. “Literally, my teams are working seven days a week, 16 hours a day.”
The same issue that has hampered efforts to stem the spread of the coronavirus—the lack of widespread testing—is also complicating research to understand its biology. For instance, just this week, New York City added more than 3,700 people to its death toll who were presumed to have died of the coronavirus but had never tested positive. “If we don’t have widespread testing, then I can’t rely on that to tell me that someone was positive or not,” says Duggal. 
If scientists are successful in identifying genes that presage the infection’s most devastating effects, they could more effectively triage the patients who need medical attention. If they get lucky, they might even uncover genes that make some people resistant, granting them the return to normalcy that so many people crave. So far, there’s no evidence for this. 
“I’m not convinced this is going to be the last one of these infectious disease crises,” Chung says. “I think what we’re trying to do is learn what we can for this one condition . . . and gain a better understanding of the immune response and generally how we fight infections.”

“This is the first study to report global distributions of HLA types and haplotypes with potential epidemiological ramifications in the setting of the current pandemic,” write the authors, from Oregon Health & Science University, Portland, and the Portland VA Research Foundation.
“HLA typing can be fast and inexpensive,” the authors write. “Pairing HLA typing with COVID-19 testing where feasible could improve assessment of viral severity in the population. Following the development of a vaccine against SARS-CoV-2, the virus that causes COVID-19, individuals with high-risk HLA types could be prioritized for vaccination.”

The Mafia is poised to exploit coronavirus, and not just in Italy

Claudio Fava, president of the regional anti-Mafia committee, described it as a "real scandal, an insult to those who lost their relatives in the pandemic."
Funerals have been banned in Italy since early March as part of a broader set of restrictions aimed at curbing the Covid-19 outbreak that has killed nearly 23,000 people as of Friday.
That the procession took place at all speaks to the power -- and the impunity -- wielded by the Mafia in parts of Italy.
Senior anti-mafia officials and researchers have told CNN that Mafia clans are already taking advantage of the coronavirus pandemic, especially in southern Italy.
They are providing everyday necessities in poor neighborhoods, offering credit to businesses on the verge of bankruptcy and planning to siphon off a chunk of the billions of euros being lined up in stimulus funds.
Journalist Roberto Saviano -- author of "Gomorrah: Italy's other Mafia," an expose of the Camorra mafia in Naples -- told CNN that "the traffickers took advantage of the [lack of] oversight of law enforcement in the ports, in the airports."
"Who was checking any more?" he asked.

Exploiting a desperate need for cash

But Mafia groups are about far more than trafficking cocaine. They are deeply embedded in the economy.
While traditional Mafia activities such as extortion may suffer during the pandemic, there will also be fresh opportunities, said Anna Sergi, senior lecturer in criminology at the University of Essex.
Franco Gabrielli, head of the Italian police, said that Mafia organizations are already deeply enmeshed in parts of the economy "that have not been blocked by the restrictions of Covid-19: the agriculture-food chain, the supply of medicines and medical equipment, road transport."
It's a view shared by Saviano.
"Funeral homes they invest in, hospital laundries. Cleaning companies they've always invested in. Good delivery companies, gas stations, this is the portfolio they've had for the past 10 years," he told CNN.
Such is the financial muscle of the 'Ndrangheta, Gabrielli said last week, that it could exploit a desperate need for cash faced by businesses it doesn't currently control.
"At the end of the emergency, the criminal associations could have polluted the economy, controlling companies previously not infiltrated," he said.
Sergi says past crises have shown that the Mafia can move money quickly outside the banking system and demand fewer guarantees than banks. Lending money to distressed companies and then gradually taking control of them is a well-oiled mafia tactic.
Nicola Gratteri, an anti-Mafia investigator and head of the public prosecutor's office in Catanzaro, told CNN that businesses like restaurants and hotels are especially vulnerable.
The last great recession, in 2008, offers a sobering comparison.
Anti-Mafia groups like SOS Impresa said that crisis turned the Mafia into Italy's largest bank. The Palermo-based group estimated in 2012 that the Mafia had 65 billion euros ($72 billion) in liquidity and described extortionate lending as "a national emergency." At the same time, many Italian banks were struggling to stay afloat and borrowing heavily from the European Central Bank.
Saviano believes liquidity will be "the center of everything" in the aftermath of the coronavirus crisis.
"The organization will come to a company in crisis and say: 'We don't buy everything, but we will give you cash in exchange for shares -- to become part of your company,'" he said.
"This is what they will do with everyone."

A parallel state

At the end of last month, video footage of a couple hammering at the doors of a bank in the southern city of Bari went viral. "You suck, the state sucks. How are we going to manage?" the woman shouts.
These are exactly the sort of circumstances the Mafia exploits. The 'Ndrangheta and other clans don't simply live off societies where they are strong. They build loyalty by supplying necessities to poorer neighborhoods and cash to struggling businesses.
To Gratteri, this is "a method to create allegiance. If we -- the state -- don't show ourselves to be efficient, what could happen is that the Mafia presents itself as a winning a model, and maybe asks for a return favor at the elections."
Zora Hauser, a researcher into organized crime at Oxford University, said that "what we are seeing -- and will see more and more as the economic and social crisis unfolds -- is Mafia groups returning to their core businesses of protection and governance."
Salvo Palazzolo, a journalist with La Repubblica newspaper, received threats after reporting on food handouts by someone with Mafia connections in the poor "ZEN" district of Palermo in Sicily.
"At this moment, Mafia families of Palermo [the Cosa Nostra] are very strong, especially in drugs and online gambling. They have a lot of liquidity," Palazzolo told CNN.
"I would say the Cosa Nostra are strengthening their control through welfare assistance to families who are in jail, and now this is spreading to all poor families. They want to show themselves as an alternative to the state."
In another district of Palermo, a local Mafia boss tried to organize a Good Friday church service in defiance of the lockdown before police intervened.
Criminologist Anna Sergi says the Mafia want the community both to like and need them.
The government is aware of the danger. CNN obtained a letter written by Interior Minister Luciana Lamorgese to regional leaders warning that criminal organizations would try to use "forms of support" to gain popularity.
As Zora Hauser puts it: "This can be cashed by the organization in different ways, the most worrying one being votes."
The government has established a 400 million-euro ($435 million) food voucher fund and allotted another 4.3 billion euros ($4.8 billion) to local mayors.
As Prime Minister Giuseppe Conte promised on March 28: "We know that many suffer, but the state is there."
But it's a daunting challenge. Saviano says that during this crisis, the mafia will try to hire the "new unemployed" as its foot soldiers. As many as three million Italians work "off the books," according to multiple surveys. One survey by the Organisation for Economic Co-operation and Development in 2017 estimated that a quarter of Italians in their early 20s were neither in employment, nor in education or training.

Stimulus bonanza

Getting Italy back to work is the government's priority. It's injecting 750 billion euros ($815 billion) into the economy.
Part of the investment is to guarantee loans to businesses -- covering more than three-quarters of a large company's borrowing. But given the massive sums being dispersed, anti-Mafia investigators worry that some of those loans, as well as other support, will go to Mafia-run businesses.
Italy also wants Europe-wide "coronabonds," basically a pooling of debt among EU member states. One German commentator, Christoph Schiltz, urged German Chancellor Angela Merkel to reject the idea, writing in Die Welt newspaper that "the mafia is just waiting for a new shower of money from Brussels."
Italian Foreign Minister Luigi Di Maio described Schiltz' comments as "shameful and unacceptable."
Saviano said: "European funds today help the Italian economy that is on its knees. And an Italian economy on its knees means Italy is at the mercy of organised crime."
It's not just Italy's problem. The Mafia's tentacles spread across Europe and far beyond. The 'Ndrangheta has a Europe-wide network for distributing drugs, often using pizza restaurants as fronts. It has also invested in property.
For now, reinforced border controls and nationwide lockdowns are choking distribution networks for drugs. But like the coronavirus, the mafia groups are no respecter of borders. "The Mafia is very powerful also in Germany," said Roberto Saviano. "They shoot less, but they are very powerful."
Earlier this month, Pope Francis prayed for "people who during this time of the pandemic, trade at the expense of the needy and profit from the needs of others, like the Mafia, usurers and others."
"May the Lord touch their hearts and convert them," the Pope said.
History, as well as current evidence, suggests he may be disappointed.
