The cluster effect: how social gatherings were rocket fuel for coronavirus | World news

On 15 February, a merry crowd wearing clown wigs and jester hats gathered in the town hall of Gangelt, a small western German municipality nestled by the Dutch border, to ring in the peak of the carnival season.
Beer and wine flowed aplenty as approximately 350 adults in fancy dress locked arms on long wooden benches and swayed to the rhythm of music provided by a live band.
During an interval in the programme, guests got up to mingle with friends and relatives at other tables, greeting each other as Rhineland tradition commands, with a Bützchen, or peck on the cheek.
A carnival committee of 11 men in red-and-white uniforms compered the four-hour event and gave speeches on topical issues. Covid-19, the virus that had been detected on German soil for the first time two weeks earlier, was not among them.
Yet the coronavirus is the single reason why the carnival session in Gangelt is now drawing close attention from scientists from around the world: seven people who walked out of the event later tested positive for the virus. A 47-year-old man who performed in the “male ballet” at the Gangelt carnival was the first person in Germany admitted to intensive care with the infection.
Gangelt is in the Heinsberg district, which is home to 42,000 people and has since had 1,442 infections and 43 fatalities, more than any other administrative district in the country. The national media has started to refer to it as “Germany’s Wuhan”.
A hundred days after a Chinese government website announced the discovery of a “pneumonia of unknown cause”, it has become clearer that the dynamics behind the virus’s rapid expansion across the globe has relied heavily on such “cluster effects”.
Each of the countries most heavily hit by the pandemic has reported similar stories of social, cultural or religious gatherings where large numbers spent numerous hours in close company – holding hands, kissing, sharing drinks from the same glass – which then turbo-charged the spread of the pandemic.
“One pattern we are seeing across the globe is that wherever there was singing and dancing, the virus spread more rapidly,” said Prof Hendrik Streeck, a virologist at the University of Bonn whose team of researchers has spent the last week carrying out the first “Covid-19 case cluster study” in Heinsberg.
“Most infections didn’t take place in supermarkets or restaurants,” Streeck said of his preliminary findings. In Heinsberg, his team of coronavirus detectives could find scant evidence of the virus being transmitted via the surfaces of door handles, smart phones or other objects.
Early theories that the virus at the carnival party in Gangelt could have been transmitted through the dishwater in the kitchen turned out to be a red herring: most guests drank their beer from bottles.
Instead, he said, transmission took place at “events where people spent a length of time in each others’ close company”, such as apres ski parties in the Austrian resort of Ischgl, the Trompete nightclub in Berlin and a football match in northern Italy.
“Mass events are a perfect opportunity for the virus, as people meet total strangers,” said Niki Popper, a mathematician at Vienna’s Technical University whose team has been developing a simulation that could help governments predict the development of the pandemic more accurately.
Instead of merely multiplying the number of daily cases by a certain factor, Popper’s example tries to account for what he calls the starting point of “local epidemic networks”.
“If you have 100 or 200 people spend enough time in a room with a person carrying the virus, then for example 20 might walk out with the new infection and, after a few days’ incubation time, pass it on to their families and workmates, let’s assume 10 more people each. Within a few days, the virus can thus multiply 200 times with only one new incident - and then continue.”

‘We shared cups’

Large parties played a key role in the pandemic’s rapid expansion not just in central Europe, but also in the US and Australia. New York may now be the centre of the US outbreak, but the traditional festivities around Mardi Gras is thought to have been a catalyst for a large outbreak in New Orleans.
On 25 February the entire city, with almost 400,000 residents, and an estimated tourist crowd of 1.4 million people from all over the world were swallowed up in a tide of music, floats, extravagant costumes, cocktails and revelry for the climax of carnival.
As is traditional in this Louisiana city on the Mississippi, gatherings and parades had begun weeks before, from 6 January onwards. Many people flung the doors of their houses open and crammed in friends, family, neighbours, strangers – all jostling, dancing, hugging, eating and drinking together.
There were no social restrictions during carnival. Only weeks later, on 20 March did the mayor of New Orleans issue a stay-at-home order for the city, with Louisiana ordering statewide restrictions two days later, closing schools and all non-essential businesses.
The first death in the city occurred on 13 March, and by 22 March Louisiana had 837 cases, of which 70% were clustered in New Orleans. On 1 April the death toll in the state reached 273.
“Mardi Gras was the perfect storm, it provided the perfect conditions for the spread of this virus,” said Rebekah Gee, head of Louisiana State University’s healthcare services division.
“We shared cups. We shared each other’s space in the crowds. People were in close contact catching [strings of] beads. It is now clear that people also caught coronavirus,” she added.
For health authorities that were still trying to track and contain individual infections at this stage of the pandemic, party clusters represented a particular challenge since revellers would often travel from and return to other regions, other countries or even other continents.
In Australia, for example, at least 30 people including backpackers became infected after attending a crowded dance party at Bondi Beach’s Bucket List pavilion on 15 March.
The party took place the night before restrictions came into force on gatherings of more than 500 people, during a weekend of mixed messages from the authorities. Australia’s prime minister was urging people not to gather in large numbers, but also saying he would go to a football game.
Many of those who attended the “Back to Boogie Wonderland – Tropicana” event, which told people to release their “inner Caribbean dancer”, later retired to local backpacker hostels with dorm rooms and shared facilities.
The Waverley council area, which encompasses Bondi and its backpacker community, has now become a hotspot for Covid-19, with many of its 140 cases linked to bars, clubs and restaurants, including the Bucket List.

Pray-ins and funerals

In many countries, partygoers and organisers have found themselves on the receiving end of a backlash. In Australia, the Bucket List’s Facebook page is full of livid comments condemning “idiots” who gathered to party. In Germany, cars with Heinsberg number plates have had their windows smashed and have been jeered as “corona spreaders”.
But the novel coronavirus did not require social gatherings to have an overtly hedonistic character to flourish. In the week of 17 to 21 February, around 2,500 worshippers gathered at the Porte Ouverte Christian church in the Bourtzwiller district of Mulhouse, in Alsace, eastern France, for one of the most anticipated events in the evangelical calendar.
They came from far and wide including from France’s overseas territories and Corsica, for a week of fasting and prayer – held annually for the last 25 years.
“During the five days, the worshippers greeted each other, pecked each other on the cheeks, and held hands, sometimes while praying during the services,” Nathalie Schnoebelen, spokesperson for the church said afterwards.
At the time, nobody thought more of it; it would be almost a month before France went into a national lockdown. A small cluster of coronavirus cases had broken out in the unfortunately named Contamines ski resort in the Alps, but most coronavirus cases were still confined to China.
“Contrary to what certain politicians have said, we didn’t ignore the basic rules because at the time there were not any,” Schnoebelen said.
It was only after the congregation at the Porte Ouverte had dispersed and several worshippers tested positive for the coronavirus that concerns were raised. Among those who tested positive were the church’s main pastor, Samuel Peterschmitt (son of Jean Peterschmitt, who founded the Porte Ouverte church in 1966).
In total 18 people from the Peterschmitt family tested positive for Covid-19. Samuel Peterschmitt’s son Jonathan admitted that worshippers had been in close confines for a week in “conditions relatively ideal if a virus wants to develop”.
By the time it had been identified as a cradle for the virus, church worshippers had unwittingly passed it on to others. A nurse who had been at the Porte Ouverte is believed to have originated a cluster that contaminated 250 colleagues at Strasbourg University hospital, where she worked.
Two retired worshippers returned to their home in the Ajaccio region of the Mediterranean island of Corsica. There are now at least 263 positive cases and 21 deaths on the island.
A delegation of five church-goers from French Guiana, led by their evangelical pastor Gilles Sax, returned to the French overseas territory on the north-east coast of South America, on 25 February. Sax said he felt ill and was “trembling like a leaf” as soon as he returned..
Other worshippers returned to cities and towns across France - Orléans, Besançon, Saint-Lô, Belfort, Dijon, Mâcon, Briançon, Paris. By 8 March, around 100 coronavirus cases had been linked to the church prayer week.
Today, Mulhouse is one of the worst-hit areas in France with local hospitals overwhelmed by patients with the coronavirus.
Christophe Lannelongue, until yesterday director general of the regional health authority, said: “It was a kind of atomic bomb that fell on us at the end of February and we didn’t notice it.”
In Italy, which has recorded the highest number of deaths as a result of the virus, a Champions League football match between Atalanta and Spanish team Valencia on 19 February in Milan has been described as the “biological bomb” that turned the north of the country into the centre of the country’s outbreak.
But scientists like Massimo Galli, the head of the infectious disease unit at the Sacco hospital in Milan, believe the spread is more likely to have accelerated in smaller clusters in the valleys around Bergamo.
“We know Covid-19 is particularly efficient at transmitting within familial groups,” Galli told the Guardian. “In osterias crowded with elderly people, it would have found a perfect environment.”
A similar gathering that brought together older and younger citizens preceded an outbreak of the virus in Italy’s south-east. On 3 March, hundreds of people gathered to pay their last respects at the funeral in San Marco in Lamis, a small city of 13,000. The 74-year-old man being buried had died after complications of what seemed to be a case of seasonal flu.
Until that day, there had been seven cases of coronavirus infection in Puglia, the heel of Italy’s boot. The photos of patients in the red zones of northern Italy, and the first fatalities in Lombardy, seemed unimaginable, much like the national lockdown that was extended a few days later across the entire country.
However, soon after the funeral, the man’s wife, who had just begun mourning the death of her husband, received a call informing her that her husband had tested positive for coronavirus.
According to Ludovico Vaccaro, head of the public prosecutor’s office in Foggia, “a postmortem revealed the man was infected with Covid-19, but the result came out only after his body was released to the family for the funeral. They should have waited for the result of the test before handing over the man’s body.”
Authorities said that, a few days earlier, the man had travelled to Cremona, in the north of Italy, to visit his daughter. Upon returning, he began to experience flu-like symptoms.
According to doctors and investigators, the man had infected his daughter and wife before he died. At the funeral, they then came into contact with dozens of relatives and friends who gave their condolences to the family with handshakes and hugs, opening a gateway for one of the first clusters in southern Italy. As a result, 110 people were quarantined, dozens of whom tested positive for coronavirus. Two have died.
The prosecutor’s office in Foggia has begun an investigation into what the governor of Puglia has called a “catastrophic error”. Behind one of the most dramatic cases of the virus’s spread is human error: if doctors had waited for the test results before handing over the body to family members, perhaps dozens of people could have avoided infection.

A loop not a curve

Thinking of the coronavirus pandemic as a process that can speed up dramatically through human clusters also affects the considerations of policymakers who are plotting a route out of the current lockdown. When governments start to relax the current social distancing measures, can they also afford to allow large social, cultural or religious gatherings to go ahead?
“The image of the Covid-19 epidemic as a curve has taken hold in the collective imagination, but my fear is that we are actually dealing with a loop,” said mathematician Popper. “As soon as restrictions are loosened, there could be new clusters that allow the spread to accelerate again.”
The story of the pandemic in Vietnam serves as a cautionary tale.
The south-east Asian country recorded its first cases of coronavirus on 23 January, a development that prompted swift action from officials. Vietnam kept many schools closed following the traditional lunar new year holiday late in January, and banned flights to and from China on 1 February. It also became the first country outside China to introduce a mass quarantine, when, on 12 February, officials isolated villages about 25 miles (40km) from Hanoi that were home to 10,000 people.
Vietnam had detected no new cases for three weeks when a flight from London touched down at Hanoi’s Noi Bai international airport on 2 March. Among the hundreds of passengers onboard was a 27-year-old Vietnamese woman returning after a trip to attend exclusive fashion week events in Europe. Along with her sister, she had sat among guests at a Gucci show in Milan, and watched Yves Saint Laurent in Paris, documenting her travels on Instagram.
The woman later told the New York Times that she had flown to Milan on 18 February and that, at the time, “no one was talking about the virus”. She travelled to Paris on 25 February, she added, and said she felt fine when she flew home that week.
Days after arriving back in Vietnam, she developed a cough and tested positive for coronavirus, becoming the country’s “patient 17”. As of 7 April, Vietnam has 245 confirmed infections. The effort to trace and contain the virus has to start all over again.

One recent analysis of Chinese data estimated the chance of death in confirmed COVID-19 cases at more than 13% for patients 80 and older, compared to about 0.15% for patients in their 30s, and virtually 0% for patients under 20. A study of early U.S. cases by the Centers for Disease Control and Prevention (CDC) had similar findings.
Partly this may be explained by the fact that older immune systems tend to be less efficient at clearing viral infections. However, that’s probably not the full story. “People in their 40s, 50s, and 60s generally aren’t hit by other viral infections, such as flu, the way they’re hit by COVID-19,” said Dr. Fred Pelzman, associate professor of clinical medicine at Weill Cornell Medicine and associate attending physician at NewYork-Presbyterian/Weill Cornell Medical Center.
Severe COVID-19 is driven not just by viral damage to cells but by a reactive “storm” of inflammation that harms the lungs and other organs. There may be changes in different parts of the immune system from aging that make the middle-aged more vulnerable to this storm than younger patients, even if they are healthy and have no underlying medical conditions. There may also be lifestyle factors, such as a greater likelihood of encountering the virus in social and work settings that middle-aged people frequent, that contribute to their vulnerability, Pelzman said.
Children can get COVID-19 infections, but are largely spared severe illness. Again, the reason isn’t clear. One suggestion from studies of other viruses, including the related coronavirus that caused the 2002-04 SARS epidemic, is that children and younger animals may be much less likely to develop an inflammatory storm when infected.
Men vs. women
Men and women appear to get COVID-19 at roughly equal rates, but in most countries men are much more likely to die of it. In Italy and Ireland, for example, males account for about 70% of COVID-19 deaths.
There is no shortage of hypotheses for this difference. One is that there are sex differences in the immune response – studies of influenza, for example, have found that older men tend to have worse outcomes than older women. Men also are more likely to drink alcohol, which weakens the immune system and increases susceptibility to pneumonia. Men are much more likely to smoke tobacco, which weakens immunity and overall lung function, primes the lungs and other vital organs for greater inflammation, and leads to greater susceptibility to respiratory infections and pneumonia. Chinese clinicians treating COVID-19 cases early this year found that a history of cigarette smoking was a very strong risk factor in predicting worse disease outcomes.
Even so, the studies needed to determine the factors underlying men’s extra vulnerability just haven’t been done yet, Pelzman emphasized.
Underlying medical conditions
Patients who develop serious or fatal COVID-19 are disproportionately likely to have at least one major underlying health condition, such as diabetes, hypertension, obesity, cardiovascular disease, asthma, kidney disease or chronic obstructive pulmonary disorder.
In some cases, the possible explanations for these links are obvious. Diabetes and obesity are associated with a weaker resistance to infections; a letter from Weill Cornell Medicine physicians published on April 17 in the New England Journal of Medicine suggested that obesity, particularly in men, was associated with treatment requiring mechanical ventilation. Asthma and chronic obstructive pulmonary disorder involve reduced lung function, and a greater susceptibility to lung inflammation; moreover, patients with these disorders often use corticosteroid immune-suppressing drugs, which reduce immunity to respiratory infections. In general, any serious underlying medical condition can make a vital organ less able to withstand the biological stresses caused by an infection and resulting inflammation.
Some researchers have suggested that common treatments for high blood pressure and diabetes may worsen COVID-19 risk, based on the fact that these drugs can boost the levels of ACE2, a cell-surface enzyme that the COVID-19 coronavirus uses to get into cells. However, there is no clinical evidence that these drugs worsen risk, and doctors generally have not advised patients to stop taking them.
Many people take non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen for chronic conditions such as arthritis. These drugs are known to stress the kidneys when taken long-term and may even cause chronic kidney disease. That is potentially a problem in the context of COVID-19 because the infection often attacks the kidneys. Some intensive care specialists have observed unexpectedly severe cases of COVID-19 in people with histories of long-term NSAID use, Pelzman said.
Having an unusually weakened immune system, for example due to cancer treatments, organ transplants, or other conditions requiring patients to take immune-suppressing drugs, is another factor that may greatly increase the susceptibility to serious COVID-19 infection – and make people more contagious during infection. Doctors have been advising those with suppressed immune systems to be extra careful to avoid potential exposure to the virus, for example by staying home, and washing hands frequently..
Race
There is emerging evidence that African Americans are disproportionately likely to develop severe COVID-19. Currently in Louisiana, for example, this group accounts for about 70% of COVID-19 deaths, although they make up only a third of the state population. Similar discrepancies are found in many other states. It is simply too early to know what accounts for this apparent vulnerability, but possible factors include African Americans’ relatively high rates of medical conditions already linked to severe COVID-19, including hypertension, kidney disease, obesity, and diabetes. Socioeconomic factors, such as poverty, access to health care and health insurance may also play a role.
Immune system ‘gaps’
Some people who seem perfectly healthy and are not considered immune-deficient may nevertheless have inherited immune system features that leave them more vulnerable than average to certain viral infections. This may be due to random genetic variation as well as ethnic background – in principle, populations with less historical exposure to coronaviruses could be more vulnerable to COVID-19. Research linking immune system gene variants to COVID-19 susceptibility is just beginning, however.
Viral dose
Doctors have long known that the amount, or “dose,” of exposure to an infectious agent can be an important determinant of disease severity. Researchers now are looking at this as a factor that possibly explains why some otherwise healthy people are hit so hard by COVID-19.
“Catching a low dose from someone who was mildly symptomatic on the subway may involve a lower risk of severe illness,” Pelzman said, “compared to catching a high dose from a very sick and highly contagious patient.”
Viral strain
SARS-CoV-2, which causes COVID-19, is a single-stranded RNA virus that has the ability to mutate quickly. Over time, and as it spreads around the world, it will develop genetically distinct strains. Some of these strains may spread more easily, or cause more severe disease. However, to date there is no evidence that the SARS-CoV-2 viruses circulating now are clinically very different from each other or that their minor genetic differences explain the range of symptom severity patients are experiencing.
Unknowns
Ultimately, researchers will need time to analyze the coronavirus that causes COVID-19 and evaluate the characteristics of patients who become infected in order to tease out the complex factors that explain why some people get mortally sick while others get only a brief fever and cough – or no symptoms at all.
Said Pelzman: “While we wish we had all the answers, we clearly don’t yet.”

Coronavirus: 30 strains have developed, most deadly in Europe | Daily Mail Online

There could be as many as 30 different strains of coronavirus, a study of patients in China has claimed.
Zhejiang University scientists studied a small number of patients with the disease and uncovered tens of mutations - 19 of which had never been seen before.
Some mutations boosted the virus' ability to invade cells in the body, others helped the disease multiply more rapidly. 
The most deadly strains were genetically similar to the ones that spread in Europe and in New York, reported the South China Morning Post

Meanwhile, the weaker strains were similar to those found circulating within other parts of the US, such as Washington State. 
The authors say their findings - based on just 11 patients - are the first to show the mutation could affect the severity of illness.
They believe the previously unreported mutations could be the reason behind Europe and New York's devastating death tolls. 
It is still unclear why the aggressive strain of COVID-19 spread to Europe and the more mild version hit large swathes of the US.
But scientists say viruses are constantly mutating to overcome immune system resistance in different populations.

THREE TYPES OF THE CORONAVIRUS ARE SPREADING AROUND THE WORLD 

Three types of the deadly coronavirus are spreading around the world - and the US is being rocked by the original strain from China.
Cambridge University researchers mapped the genetic history of the infection from December to March and found three distinct, but closely related, variants.
Analysis of the strains showed type A - the original virus that jumped to humans from bats via pangolins - was not China's most common. Instead, the pandemic's ground-zero was mainly hit by type B, which was in circulation as far back as Christmas Eve.
Results showed type A was the most prevalent in Australia and the US, which has recorded more than 400,000 COVID-19 cases. Two-thirds of American samples were type A - but infected patients mostly came from the West Coast, and not New York.
Dr Peter Forster and team found the UK was mostly being bombarded with type B cases, with three quarters of samples testing as that strain. Switzerland, Germany, France, Belgium and the Netherlands were also dominated by type B.
Another distinct variation, type C, descended from type B and spread to Europe via Singapore.
Scientists believe the virus - officially called SARS-CoV-2 - is constantly mutating to overcome immune system resistance in different populations.
It comes on the heels of studies that claim the US was hit by two different clusters of the coronavirus, with type A dominating the West Coast and the deadlier type B in New York. 
Experts say the type A outbreak was spread to the US from China, where as the crisis in New York likely came from Europe - which was also rocked by type B.  
For the latest study, published on the pre-print service medRxiv.org, the team analyzed viral strains from 11 Chinese coronavirus patients.
The team, conducted by Professor Li Lanjuan and colleagues, tested how effectively the virus could infect and kill human cells in the laboratory.
Viral load - the amount of the virus - was assessed in all the cells after one, two, four and eight hours, as well as the next day and 48 hours later. 
And the experts also looked at the cytopathic effects - whether the virus structurally changed the cell during infection - up to three days after the experiment. 
The most aggressive strains created up to 270 times as much viral load as the least potent type, according to the results.
And the strains that produced the highest viral load led to a 'higher cell death ratio', Professor Li and her team revealed.  
Writing in their paper, the team said: 'Our results show the observed mutations can have a direct impact on the viral load and CPE.  
'This finding suggests the observed mutations in our study... can significantly impact the pathogenicity (the ability to cause disease) of SARS-CoV-2.'

WHAT DID THE STUDY FIND?

These mutations had also been seen in several hard-hit European countries such as Italy and Spain - before spreading to the US epicenter New York. 
However, some of the milder mutations were the varieties largely found in the US, including Washington state, which could be the strain that shut down Wuhan, where the virus originated. 
But the scientists admitted that the 'full mutational diversity of the virus in Wuhan in the early days is still unknown'. 
The researchers warned that just because the mutations were milder, it didn't mean there was a low risk of mortality.
Ten of the 11 patients who were studied had clear connections with Wuhan, the city where the pandemic began in December.
All of the patients - aged between four months and 71 - recovered. Eight were men, and three were women.  
Two patients in Zhejiang, one in their 30s and one in their 50s, became severely ill after contracting weaker strains.
Although both of the patients recovered, the older patient required treatment in a hospital's intensive care unit.

HOW DOES CORONAVIRUS HIDE IN THE BODY?  

The SARS-CoV-2 virus has a large number of spikes sticking out of its surface which it uses to attach to and enter cells in the human body. 
These spikes are coated in sugars, known as glycans, which disguise their viral proteins and help them evade the body's immune system.
'By coating themselves in sugars, viruses are like a wolf in sheep's clothing,' explained Professor Crispin.
The coronavirus has a relatively low level of sugar shielding.
The lower glycan density means there are fewer obstacles for the immune system to neutralise the virus with antibodies.  
Researchers detected about 30 mutations in total. About 60 percent of them, or 19, were new. 
The authors say that patients with COVID-19, the disease caused by the virus, have been receiving the same treatment at hospital regardless of what strain they have. 
They say that strains may need different efforts to fight the virus, which undergoes one mutation a month, scientists say.
'Drug and vaccine development, while urgent, need to take the impact of these accumulating mutations...into account to avoid potential pitfalls,' the researchers said.  
Worldwide, more than 2.45million people have been infected and more than 168,000 people have died.
In the US, there are more than 771,000 confirmed cases of the virus and more than 41,000 deaths.

A second strain struck the West Coast and likely arrived later from China, despite the fact that a man who traveled back to the US from China was the first identified American infected with COVID-19.

Separately, Cambridge University scientists found that there are three distinct strains of the virus that account for most of the world's cases.
It's now quite clear that coronavirus first made the jump to humans from anteater-like animals called pangolins after getting its start in bats. 
As it has spread among humans around the world, the virus that causes COVID-19, SARS-CoV-2, has mutated into different strains - although not as quickly as some viruses evolve. 
Many scientists have delved into the genomes of these viruses to try to work out their ancestry and how each has traveled and spread.

Researchers at <a href="http://Nextstrain.org" rel="nofollow">Nextstrain.org</a>, traced the virus strains, coded in bright colors on an interactive map as they've criss-crossed the globe and multiplied in number from the first iteration of coronavirus in China.  
The authors of the new Cambridge University study dubbed the first strain to hit humans 'type A.' 
But the original didn't linger long in China, instead jumping to and taking hold in Japan, Australia and the US. 
Instead, a mutated version of the virus - type B - arose and became the dominant spread in China, before moving on to Europe, South America and Canada. 
Still a third strain, type C, became the dominant version of COVID-19 in Singapore, Italy and Hong Kong, according to the Cambridge analysis of viral genomes in samples from coronavirus patients.

Coronavirus wreaks havoc on New York City's public housing

NEW YORK — The coronavirus preys on vulnerabilities, and that has made New York City's public housing system and its tenants unfortunate targets.
The results have been deadly.

As the virus sweeps through the roughly 174,000 apartments overseen by the New York City Housing Authority, it is threatening the economic picture of an agency just starting to find its fiscal footing and exacerbating inequalities that have long plagued low-income communities. Many NYCHA tenants have pre-existing medical conditions. Many are seniors or frontline workers who risk their health while on the job and then come back to apartments cramped with family members sheltering in place.
Residents and officials interviewed by POLITICO said that with health care systems overwhelmed, tenants are dying in their homes every day. Because the city has not been testing the deceased for Covid-19, the cause of each particular case is unclear. But the increased count is palpable in the data.
“So many people have died this week,” said Lisa Kenner, resident association president at Van Dyke Houses in Brooklyn. “It’s enough.”
Nearby Woodson Houses has lost six seniors. A few blocks away at Glenmore Plaza, the former resident association president died in her apartment last Friday morning. She was not taken away until the next day, according to Kenner.
She said 10 residents have died in their apartments at Van Dyke. In one case, the bodies of a mother and son were discovered in their unit only after the smell prompted neighbors to call city officials. Eventually, the National Guard came to remove the bodies, and then cleaning crews passed through the home. But the odor of death lingered.
“NYCHA has always struggled,” said City Council Member Alicka Ampry-Samuel, chair of the Council’s public housing committee, who represents Kenner's district. “Unfortunately because of this crisis, they are struggling even more.”
New Yorkers have been dying in their homes at around 10 times the usual rate, according to city statistics. Several officials have posited that the majority of these cases stem from low-income residents who cannot afford access to the traditional health care system or are frightened to seek assistance because of their immigration status.
"Residents are experiencing challenges, suffering, hardship, fortitude and survival, along with the entire community," NYCHA spokesperson Barbara Brancaccio said in a statement. "We are heartbroken to hear that we have lost members of our NYCHA family."
Many public housing tenants lack a primary care physician and typically use the emergency room as a first stop for health care. Hospital admissions, however, have been focused on sparing beds for the gravely ill. And while the mayor has said that anyone can call 311 and get connected with a doctor, Ampry-Samuel said the makeshift system is not reaching enough of the city’s vulnerable population and was not designed with them in mind.
“We could have prevented some of these deaths happening in the home if our leaders in the executive offices were talking directly with local elected officials,” she said.
The medical issues are being compounded by economic woes now rippling through every level of the public housing organization.
More than 45 percent of NYCHA families are working. But as unemployment claims reach staggering levels, officials expect public housing tenants to be hit hard. Not only will that strain household finances, but it will further imperil the economic standing of an agency already hollowed out by decades of underinvestment.
Around half of NYCHA’s annual operating costs are covered by $1 billion in rent received from tenants, who use federal vouchers to supplement their payments.
“If folks are out of work and stay out of work, we will need more voucher money and more operating subsidy,” NYCHA Chair Gregory Russ said in an interview.
NYCHA has received $137 million in operating money from the latest federal appropriations bill, enough to cover short-term costs, Russ said. But as the broader economic picture continues to sour, the odds grow that more cash will be needed — especially because several revenue-raising strategies the de Blasio administration was banking on have also been scrambled by Covid-19.
March was supposed to be a pivotal month.
A working group that was formed to hash out controversial plans for market-rate apartments and private management at Fulton Houses in Chelsea was set to unveil its recommendations, which would then set the benchmark for similar endeavors down the road. NYCHA hopes that various combinations of infill development and outside management contracts would cover around $15 billion worth of capital repairs across its entire portfolio. Both initiatives have prompted opposition from some elected officials and tenants. And the concept of new development has proven especially caustic.
Though consensus was far from guaranteed, city leaders and interested parties were leaning heavily toward scrapping the most hot-button issue at Fulton Houses: tearing down two existing public housing buildings. The group had instead turned its focus toward alternative development scenarios that would slot new buildings into existing open spaces elsewhere on the grounds, according to several people with knowledge of the meetings. But the sessions have stopped. And it is unclear when and if the plan will move forward.
Russ was also on the verge of unveiling a sweeping capital plan that had been in the works since last year. The agency planned to outline roughly $40 billion in repairs that, if funded, would bring buildings up to a minimum state of repair and allow the authority to finally emerge from crisis mode and start operating more akin to a normal landlord. The release of the plan has been tabled for now, though officials may break it out if a federal infrastructure bill materializes in Washington.
“If they are going to go that route, we have a program of scale and size that would be ideal," Russ said. "And we would make our voice heard."
NYCHA officials said recently they were on track to meet their goal of enrolling 62,000 apartments into the federal Rental Assistance Demonstration program, an initiative that transfers management of certain complexes to private developers who then make capital repairs. New projects requiring public hearings, however, have been put on hold as the authority more generally pares back its operations in response to Covid-19.
Development staff are cleaning buildings, performing emergency repairs and doing critical work related to lead and mold, the authority said. But with the onset of the coronavirus, workers have stopped doing annual inspections of properties required by the U.S. Department of Housing and Urban Development and the authority is limiting lead inspections to the universe of properties already known to have both lead paint and children under six years of age. As many tenant interactions as possible are being conducted remotely.
That has thrown the city out of compliance with a federal settlement agreement. For the time being, NYCHA is working with Bart Schwartz, the federal monitor, on an interim agreement that will prioritize what can be done until full operations resume and will, for example, likely delay when the authority must submit a reorganization plan.
NYCHA has also hired several outside contractors to clean and disinfect buildings, and has been partnering with food banks to distribute meals at pop-up events. In some cases, though, tenants have had to take the lead.
Back in Brooklyn's Van Dyke Houses, Kenner said she had to turn to the nonprofit City Harvest to deliver meals to seniors after an order placed through the city for Meals on Wheels delivery never arrived — a problem affecting elderly residents across the city. And she tapped into her federal money for the resident association to purchase 2,000 masks — something NYCHA has encouraged — after seeing seniors and other residents leaving their apartments without protective equipment to get food and medicine. Until this week, frontline caretakers and maintenance staff at her complex were similarly working without protective gear.
She said that NYCHA has been effective at communicating with residents about social distancing and what to do if they cannot pay rent. But as the burdens pile up, her neighbors are struggling.
“It’s really getting to people mentally,” she said.

The 75th Anniversary of the WW2 end, with the deeper backdrop of the German and the European general and military Histories was the occasion for this commemoration and the neurotic Undoing of the German defeats. The FBI information about the U.S. white supremacists' as of early (January 2020) knowledge and the social media discussions on this subject appear to confirm this line of thought. Apparently, there might have been the involvement from the Russian Nationalists also, who very likely, in turn, used the help from the Russian private Spetsnaz, which is on the payroll of the Russian Jewish Mafia, and who faithfully informs the GRU. 
This hypothesis is as good as any other (there are no others with this scope) or better, since it is comprehensive and is rooted in historical facts.

At least 100 UK healthcare workers have died from coronavirus, figures show | World news

At least 100 health and care workers have died of coronavirus, a nursing website has said, amid growing concerns about a lack of personal protective equipment (PPE) for those working on the frontline during the coronavirus pandemic.
The number of nurses, doctors, porters and members of other professions who have died has been catalogued by Nursing Notes, a platform run by nurses for others in the profession.
They announced that at 2pm GMT a milestone had been reached with at least 100 health and social care workers now believed to have died. The website has been recording all deaths since the outbreak began, verifying all the information they receive using a number of independent sources and permissions sought from family, friends or colleagues.
The government has publicly verified 27 deaths of NHS staff from Covid-19 during the pandemic.
Among those who have died are Linda Clarke, who worked as a delivery suite coordinator at the Royal Albert Edward Infirmary. She had worked in the maternity service for 30 years, with trust chiefs saying she had “brought many new lives into the borough”.
In a statement, Silas Nicholls, the chief executive at Wrightington, Wigan and Leigh NHS trust, said: “We can sadly confirm that a member of our staff, Linda Clarke, tragically passed away on 17 April. Linda was 66 and worked in our maternity service for 30 years, bringing many new lives into our borough and caring for expectant mums in our community.”
Ade Dickson, a mental health nurse for the Barnet crisis resolution and home treatment team, also died. A spokesperson from Barnet, Enfield and Haringey mental health trust said: “We are very sad to confirm the loss of our colleague and friend Ade Dickson due to coronavirus.”
They added: “Ade was a highly respected colleague who will be deeply missed by his family, friends, trust staff and patients. We ask that you please respect their privacy at this difficult time.”
The Guardian has also been recording the deaths of NHS staff and has written tributes for 81, with the number continuing to be updated as more names come in.
Over the weekend, hospital leaders have directly attacked the government for the first time during the coronavirus crisis over the shortage of PPE after a desperately needed consignment of surgical gowns that had been announced by ministers failed to arrive.
In an unprecedented intervention, which hospital leaders privately say is the result of “intense frustration and exasperation”, the organisations representing NHS trusts in England urged ministers to “just focus on what we can be certain of” after weeks of “bitter experience” with failed deliveries.
The NHS Confederation and NHS Providers spoke out amid continuing alarm that shortages of equipment will soon have disastrous effects on the frontline, with representatives of intensive care staff warning on Sunday that the critical shortage of PPE could lead to some people refusing to continue working there.

Since the novel coronavirus was first identified nearly 4 months ago, the world has come a long way in understanding how the virus infects people and how it spreads among populations.
Over 2.43 million people around the world have contracted COVID-19, the disease caused by the virus, and there are more than 1.6 million active cases.
However, ongoing research on and about these patients has revealed that many of our best original assumptions about the virus weren't fully accurate — or in some cases misguided.
After China confirmed the first case of the mysterious "pneumonia-like" illness at the end of December, for example, it turned out someone else likely started spreading it there in November.
